Worldwide, hospitals continue to be confronted with a nursing shortage problem, which has been a concern for decades. According to Whitney (2018), there is a deficit in the nursing workforce related to the age of the current nursing workforce, the need for schools to enroll more students at an accelerated rate to meet the projected demand, and a shortage of nursing faculty and clinical sites. Moreover, the population is aging, and long-term care needs are increasing. Patient care can become too overwhelming as one age. The solution to this problem can be found by employing older nurses in the facility’s education and non-direct patient care areas as resource personnel (Marquis & Huston, 2017).
With more experienced nurses retiring earlier, younger nurses will need to gain exposure to the valuable experience and knowledge that older nurses have accrued during their careers. The availability of clinical preceptor sites for nursing students also contributes to a need for more nurses. Because many nursing students apply at the facilities where they do their preceptorships, hospital systems that partner with nursing schools can benefit financially and as employers. As a result of staffing shortages, higher patient needs and pressures, and potential violence, nurses are feeling burned out and are seeking employment outside of healthcare. There is a need for changes within the nursing industry. There are several issues related to nursing, but there are also options, and nurses tend to go where they are well-paid and treated.
To improve the quality of service, facility leadership should pay attention to and listen to the staff when they talk about issues. This is the biggest issue in the region I practice; nurses are not being incentivized or paid anywhere near what they are worth for the knowledge and experience they bring with them. Nursing salaries in Oklahoma are among the lowest in the nation. RNs in Oklahoma earn an average salary of $64,800 a year. Accordingly, Oklahoma nurses make an average hourly wage of $31.15. Meanwhile, the national average for RNs is $77,460. In terms of an hourly wage, this is approximately $37.24 an hour annually (Registered nurse salary in Oklahoma (2021): Trusted health).
Lack of mental health support and understanding is another issue plaguing nursing. We have had two attempted suicides by two nurses at my current facility. I can only speculate as to the reason, but there is no doubt that the current working conditions of nursing were not one of the possible and probable reasons for them to attempt such a permanent act. Mental health strains result from long shifts, staff shortages, and a lack of resources, so healthcare organizations must support nursing staff. Establishing a mental health-positive workplace culture that provides mental health resources to destigmatize mental health issues is essential. Nurses’ mental health can be enhanced and protected by peer support groups and self-care strategies, enabling them to perform at their best (2022). Happy and healthy nurses are essential in creating and maintaining happy and healthy patients. A healthy work environment contributes to the retention of nurses by creating a positive work environment. It is important to note that as nurses are retained, staff shortages will decrease, thus positively impacting patient satisfaction and safety.

Consequently, this endeavor will not only aid in cost savings but also lower death rates. As key players in the healthcare system, nurses can inspire individuals by urging them to seek out different services that can serve as preventive measures and by making recommendations that are supported by the available data. A major difficulty is providing for and financing the medical care of patients with long-term health issues. The Centers for Medicare & Medicaid Services are now in charge of several programs under the Affordable Care Act that attempt to increase quality and reduce costs (Salmon & Echevarria, 2017). To collaborate with doctors as patients become more complicated, nurses must retain proficiency in leadership, health policy, systems improvement, research, evidence-based practice, teamwork, and advanced education. Patients’ health concerns are increasing despite the nursing shortage, which is a challenge in the nursing practice. It has been determined that more nurses will be needed within the practice (Tamata et al., 2021). The nursing shortage is one of the challenges that different events have contributed. It has been determined that the number of older people is increasing globally, contributing to increased concerns for patients who require critical care (Tamata et al., 2021). Statistics have also indicated that in the future, there will be an increasing number of individuals 65 years and above. These are among the main factors contributing to the worsening nursing shortage in healthcare (Haddad et al.2020). Another event contributing to the nursing shortage is the increasing number of nurses retiring due to old age. This is a fact that should be considered to be impacting nurses.
There are also events that have contributed to the nursing shortage, such as adverse environmental impacting factors. This includes fatigue within the workplace, contributing to increased turnover. The rate of health professionals that enrol as nurses have also been low because of the increased number of patients and an example of unemployment whereby there is exhaustion. Internal challenges such as burnout and low job satisfaction also attributed to increased turnover of nurses from the health practice. The nurses, therefore, end up taking longer time to provide care within the practice. In some cases reported within the health practice, violence has also contributed to the nursing shortage (Haddad et al., 2020).
The nursing profession is working towards addressing the nursing shortage challenge differently. This is associated with the introduction of solutions to increase the number of nursing facilities providing education. Additionally, it is the ability to have better transitions within the academics of the health professionals. Training programs have also been initiated at different levels to establish more knowledge on addressing patients’ health issues.

The changing trends and healthcare demands implore nurses to prepare and position themselves strategically to offer quality care as frontline care providers. The National Academies of Sciences, Engineering, and Medicine in their report “The Future of Nursing 2020-2030: Charting a Path to Achieve Health Equity,” highlight the critical role that nurses play and will continue to play to help the nation attain better health for its population, including reducing costs and emphasizing primary care (Hassmiller, 2021). The report presents eight practical strategies that the nursing profession can employ to advance health equity. The purpose of this paper is to discuss the work of the academies, the role of state-based coalitions, especially in Florida, to help nurses attain health equity.
Work of National Academies of Sciences, Engineering, and Medicine
In this work and report, the National Academies of Sciences, Engineering, and Medicine worked with different stakeholders to examine the current trends in care provision and the need for stakeholders to develop strategies to help the nation attain health equity. The work of the Committee on the Future of Nursing 2020-2030 considered the critical role of nurses, especially in the backdrop of the unimaginable situations of the Coronavirus disease (COVID-19) pandemic, and trends that will inform care delivery over the decade ahead. The committee developed the report after the Institute of Medicine’s 2010 report that demonstrated the critical need for nurses to prepare to meet the rising care demand among the diverse population (Hassmiller, 2021). The report develops eight strategies that will help prepare and train a nursing workforce to offer care, promote health and wellness among nurses, individuals, and communities as well as address the systemic inequities which continue to fuel wide and persistent disparities in health.
The report notes that building on strengthened nursing capacity and expertise will help nurses create and contribute effectively to equitable health and health care systems which can work for all people. The work of the committee emphasizes the critical role of the nursing profession in addressing the complex work of attaining alignment in public health, health care, and social services as well as public policies aimed at elimination of health disparities and attainment of health equity.
Trends in Nursing Workforce Influencing Nursing Education to Meet Professional Needs
In its report of 2010, the Institute of Medicine (IOM) highlighted the need to nurses to attain higher levels of education and training by having an education system and practice environment which support seamless academic progression. The implication is that trends in nursing workforce are changing nursing education curriculum to equip nurses with the relevant skills, attributes and knowledge to offer quality care to diverse patient populations. The primary goals of nursing education entail preparing nurses to meet diverse needs of patients, function as leaders, and advance science that offers benefits to patients and their ability to deliver safe, quality care (Kleinpell et al., 2022). Further, nursing education should transform in various ways to prepares nurses who have qualities of working collaboratively and effectively in multidisciplinary and inter-professional teams in complex and evolving health care system in different settings.
One core trend in the nursing workforce is increasing diversity with nurses of varying ages and social backgrounds working in same settings to offer care to also a diverse patient population. The increased diversity in nursing is also leading to different opportunities for specialty practice as nurses specialize to meet the needs of different populations (Stephens et al., 2021). The aging nursing workforce of baby boomers’ generation must work together with younger generations to deliver quality care. Further, nurses from diverse backgrounds have diverse perspectives and skills as well as cultural competencies that are important for teamwork.
The second trend in nursing workforce is the increased responsibilities and assumption of leadership roles. Nurses, especially those in different specialty and with managerial roles, must acquire more skills and attributes to help them deliver care to patients (Hassmiller, 2021). Leadership roles and being primary care providers mean that nurses have moved away from just the care and bedside roles and are now decision makers and policy implementers. Further, the increased need for technology-savvy nurses implies that nurses should be prepared to use different equipment and devices to help them improve care delivery to meet diverse needs, particularly the use of telehealth to reach patients in remote locations like rural areas and increase accessibility.
Nursing education should adapt to meet the evolving needs of the nursing profession in various ways. As stated, nurses need lifelong learning through an education system that recognizes and appreciates seamless learning to prepare them effectively to meet changes in patient care management (Zaccagnini & Pechacek, 2019). The nursing education is adapting to meet these needs by leveraging increased use of technology to offer more access to nursing students, especially those pursuing advanced education. Nurses can pursue advanced courses online while they offer services to patients (Jeffries, 2022). Secondly, redesigning of a curriculum that integrates new trends and practices is critical to preparing nurses to meet these needs. The education system is also leveraging new trends like integration of evidence-based practice to prepare nurses as well as offering leadership courses.
Role of Nurses in Leading Change
Nurses are patient and nursing professional advocates whose main role is to ensure quality care delivery. As change agents, nurses play a critical role in formulating and implementing health care policies aimed at improving care delivery. They also offer patient education and collaborate with other providers to offer quality and patient-centered care (Kleinpell et al., 2022). They also promote primary care in different populations to ensure that health care transformation attains health equity.
Role of State-Based Action Coalitions
The transformation of health care as depicted by the National Academies’ report requires effective implementation at state levels spearheaded by the action coalitions. These coalitions are led by nurses who partner with other stakeholders to drive local changes by working to transform health care using nursing and building healthier communities. These coalitions mobilize nurses, health care providers, consumers, and educators as well as businesses to improve nursing on different areas or fronts. These coalitions envision a healthcare system that allows nurses to contribute to full extent of their abilities and expertise.
The Florida Action Coalition focuses on provision of leadership to advance the nursing profession to improve access to safe, and high quality healthcare. One initiative by the state action coalition is the enactment of new law to increase access to better health care option. Through the coalition’s advocacy, over 21 million residents of the state can now access care from advanced practice registered nurse (Florida Action Coalition, 2022). The second initiative is the recognition of innovative nurses where the action coalition awards organizations up to $25,000 for progressive work that focuses on improving the role of nurses in building a culture of health and promotion of health equity.
These initiatives advance the nursing profession by demonstrating the need for nurses to prepare effectively to meet the diverse patient needs in different settings through specialty training and education. Secondly, the initiative on health promotion illustrates the critical role that nurses play in health promotion and the need to recognize them as primary care providers (Florida Action Coalition, 2022). The initiative to reward organizations advances the nursing professional by showing the core role of nurses in shaping and building an organizational culture that is important in care provision.
Barriers to Nursing Practice in Florida
A core barrier to nursing practice in Florida is the lack of recognition of nurses as primary care providers. While nurses have full practice authority and can practice independently in the state, they are still not recognized as primary health providers. Advanced practice registered nurses cannot practice independently in Florida until they accumulate at least 3,000 hours of experience while under the supervision of a physician (Florida Action Coalition, 2022). This requirement is a barrier to nursing practice.
The state action coalition continues to advocate for a reduction in the requirements for full practice authority for nurses. The coalition and nurses’ associations within the state advocate for a reduction in these provisions to ensure that more nurses can practice to alleviate the current physician shortage. The nursing advocates continue to lobby and develop action plans to reverse the current laws to allow increased practice.
The future of nursing is bright as nurses become primary care providers in different areas because of reforms and trends. The report by the National Academies of Sciences, Engineering, and Medicine show that nurses will play a critical role between now and 2030 to address health equity across the country by reducing the current disparities. State action coalitions like the one in Florida will play a critical role in helping nurses and stakeholders develop right interventions to meet the healthcare goals and attainment of health equity.

Currently the nursing profession is filled with many nurses that are very close to reaching retirement age. Whitney (2022) writes that in the year 2013 more than half of nurses in the United States were at least 50 years old. Currently there are not enough nurses to backfill those spaces. Nursing programs in universities and community colleges are unable to educate, train, and produce enough qualified new graduate nurses due to the fact that is shortage of nursing school faculty (Whitney, 2022). Nurses work long hours in high-stress environments that can lead to burnout. With an aging population that is living longer and with more comorbidities, it is expected that the nursing shortage will be worsened (Bakhamis et al., 2019). Not addressing the problem only exacerbates the shortage. When staff are unsatisfied and burned out they leave the profession, increasing the staffing shortage and increasing the workload for nurses that remain in the profession.
To combat the problem of low nursing retention numbers, hospitals and employers are needing to come up with ways to build nurse resilience and improve working conditions. Research by Duru et al., (2022) found that employers could be successful in retaining nursing staff by recognizing nurses for giving quality care, providing tuition reimbursement, increasing pay for additional duties such as precepting, and adding sign-on/retention bonuses. Creating an environment that allowed for shared governance has also been shown to improve retention (Duru et al., 2022). Through shared governance, the nursing profession has a seat at the negotiation table and that can have a positive impact on job satisfaction and retention numbers.
