Working as a family nurse practitioner has developed my interest in emergency care and this has necessitated my interest to enroll in advanced studies at Walden University to specialize in the same. Specifically, my concern is directed to patients with difficulty to move due to paralysis or half-paralysis and I look forward to addressing their concern in the emergency room (Gainotti, 2021). In order to access information relevant to this area of choice, I select to use an electronic database found at Walden University’s library. While doing my online research, I had difficulty in choosing the right online database in the library due to multiple options. However, since the university’s online library provides different options, I had to review the best database that gives good articles based on time limits and one that meets the criteria for best peer-reviewed materials (Justesen et al., 2021). Based on this premise, I chose CINAHL Plus with full text as the best database for online searches at the university’s library.
Despite being free, finding this database was a bit challenging especially due to multiple online options that are available in the online university library which then meant that I had to limit the search process to modes and expanders in order to get the reference above (Rao & Moon, 2021). On a positive flip-side, I find the database easy to navigate as I was able to narrow down my searches to specific words and parameters. Therefore, the CINAHL database can be used by colleagues at the university and I recommend it as the primary tool where they can look for information pertaining to area of study. Other than providing an easy-to-use platform, the database has an option for advance searches and can also be used by colleagues to cite references (Justesen et al., 2021). Moreover, the database can be used by colleagues to search for a wide range of topics in health sciences and nursing.

I’ve decided to pursue a career as a psychiatric/mental health nurse practitioner. After 28 years of working as a mental nurse, I made the decision to advance my career. Next, I selected the “peer-reviewed” link on the left and entered the keyword “psychotherapeutic,” I then restricted my search. A list of articles from the Thoreau/EBSCO search engine appeared, and I chose this particular one.
Finding this peer-reviewed article was not difficult for me. The library database at Walden University is quite trustworthy. Yes, I would recommend this database, because it is accessible, and the full articles are available for usage. “A good database should begin with valuable metadata and data”, (Siyavula. n.d.). Criteria used for selecting a good database is using one that is a reliable monitoring and alerting system, (Guo, 2022). This database will be very helpful to my colleagues for the reasons stated above. My search was able to be limited by a large number of alternatives. The Walden library’s EBSCO database has excellent themes and a big selection of articles, therefore I would definitely recommend it.

Markowitz et al., report that as practitioners, the differences between treating patients with psychotherapy via video or phone rather than in person may be alarming. The researchers reported concerns that these forms of therapy can impede emotional engagement and lead to an under-display or underreporting of symptoms or behaviors by the client (Markowitz et al., 2020).
Regarding the article you found and coming from a place of wanting to learn more about suggestions or strategies for improving database searches, I have a question for you. I could not find access to the full text, which I feel happens often for me. Do you have any tips on how to do this when searching in a database? I have honestly encountered this issue a few times for articles I would like to read and utilize.
I was able to find something called ResearchGate, and I was able to make an account. I requested permission to read the full text, but I do not know how long this will take. When I created my account with ResearchGate, it brought up a few prompts to verify my identity. There were many articles by several people who go by M. Ham, and I was asked to select myself as the author of the work in question or decline that I was the author. It was definitely something you could skip, but I got a kick out of seeing how many M. Ham’s that exist and wondered if I would ever have research published.
This inspired me to learn more about ResearchGate, an investor-funded start-up and global community to connect researchers who share and put forth science (ResearchGate, n.d.). One of their dreams is that world leaders look to scientific advisors before corporate donors, and I feel that my heart and professional goals resonate with that. Have you heard of ResearchGate?

Thank you for your post. I totally agree with you about the easy process involved in finding these peer reviewed articles at Walden University Library. I am glad you did not face any dificulty in finding these resources at the Walden Library due to it’s easy access and user-friendly nature. One can see that the Walden Library was built with students in mind. Being a student is hard enough and having such resources as support go a long way. I really appreciated the study of how you looked up your peer-reviewed article in the Walden database. I’ll have to make an effort to follow your instructions step by step. I thought this discussion forum was quite beneficial because it helped a great deal. It is a genuine academic and professional networking initiative geared toward success. I’m grateful. As a fellow future PMHNP, your article is very relevant and frightening to me. From what I gathered from reading it, it seems like if PMHNP’s want to have control over what type of work they do/how they practice they need to develop their skills independently. “However, PMHNP programs frequently rely on other professions (psychology and social work being the top two) to provide psychotherapy practicum experiences” (Wesemann et al., 2022). To me, that seems like nurses are going to have to prove their value in order to maintain their place in the future of mental health as the practice evolves. Another thing that scared me is how difficult it was for me to find the article that you cited. You cited it perfectly, but it took me longer than it should have to find the full thing. The reason for this is kind of embarrassing considering I just did this earlier this week. Once I reoriented myself to the Library’s resources I was quickly able to find what I needed.
I also found reading your post very helpful. The way you broke down how you can limit your results in several ways is super helpful and intriguing to me. I mentioned in my post the need for any nurse’s using this program to know how to tell the difference between good and bad articles quickly. Using these alternatives could help a nurse search for specific topics and exclude what they do not want. I will look at it more later today, but I wonder if you can search by topic AND type of research/study performed. That could be very useful.

You find something you enjoy in life, and you chase it. That is what I have done by deciding to further my nursing education at Walden University. I not only have a passion for caring for others, but I particularly have a passion for providing comfort to terminally ill patients. My position as a hospice nurse has been the most rewarding experience in my nursing career yet. Time after time, I provide families with comfort and support when they need it the most. I bring this to attention because I often provide families with education. At the end of life, many changes cause concern to families, especially in the home setting. My job is to provide facts about expectations during a very uncertain time in an individual’s life to comfort the patient and their family.
Symptom control drives the plan of care for our hospice patients. This significant detail led me to research a medication we commonly use to comfort patients in their final days. Morphine treats a variety of symptoms in hospice patients when not contraindicated. Morphine is more widely known for its ability to decrease pain levels. As a hospice nurse, I have observed the use of this medication for reducing pain, but I have also witnessed doctors prescribing this opioid to control breathing symptoms.
For my research regarding this topic, I utilized the Walden Library. I selected the “nursing” category on the library homepage to narrow my search. After choosing this topic, the library generated a list of databases available for research. From previous assignments, I have some knowledge of the listed databases. I have experience with the CINAHL database, specifically, so this is where I began my search (Walden University Library, 2022). My goal was to locate evidence-based information to back up the use of morphine for breathlessness or shortness of breath in terminally ill patients. It is important to me to provide families with education regarding the benefits of morphine, besides pain relief. Having the research to back up my field experience is what I need to continue to make a difference for patients and their suffering.
To my surprise, finding information regarding the use of morphine for shortness of breath was reasonably straightforward. In further detail, I researched a specific population of patients. Very commonly, cancer is the primary diagnosis in hospice patients. The article I found targeted results on the effects of oral morphine on breathlessness in cancer patients specifically. In further detail, the article I reviewed focused on the success of morphine for dyspneic patients from multiple sources of etiology. While lung cancer was the primary diagnosis for the patient population in the study, no patient health history will be identical. This study went beyond evaluating the response from different patient backgrounds and focused on personal bias toward morphine treatment. Like any part of health care, most patients or families have a preconceived idea of whether they agree with a course of treatment. The common stigma regarding morphine was addressed in this study and made reviewing it personally intriguing.
Like many studies conducted, this article had its weaknesses. This study lacked a control group but attempted to compensate for this downfall by having a large population and evaluating patients’ preconceived ideas about the treatment. The study used multiple tools and approaches to reduce bias. Subjective tests for the patients, in the form of questions, were implemented to determine the results. Some scales included in their research were the Cancer Dyspnea Scale (CDS), Visual Analog Scale (VAS), and the Numeric Rating Scale (NRS). While the study was not able to completely fix the dose of morphine given to all patients, they succeeded in setting a quantity of 5 mg for all the patients in question except for 2. This made the treatment almost unvaried.
I could go on about the positives and negatives of this study in length. One last detail I wanted to elaborate on was the choice to only evaluate the change in patient condition after one dose of morphine (Takahashi et al., 2019). This was an eye-opener for me. Many patients require drug titration, regardless of the medication, to see the full beneficial effect (Caffrey & Borrelli, 2021). While the fixed dose for morphine eliminated a great deal of bias in the study, it also limited the researchers’ ability to evaluate how effective morphine may be for each patient honestly. Adding evaluation for further response with additional dosing or titration of the dose would have been something I would have liked to see in this study.
Positively, this study was still able to conclude that oral morphine successfully reduced dyspnea based on the population evaluated and the tools utilized (Takahashi et al., 2019). I recommend this article for review by my colleagues, and I now have evidence to provide to families needing reassurance for using morphine outside of pain control. In the material resources for this course, I was encouraged by a faculty staff member at Walden to find my passion and use it to fuel my desire to engage in research. Researching something I am passionate about allowed me to enjoy this assignment and use it to continue to make a difference in my community (Walden University, 2018).

My practice area is family nurse practitioner and I am concerned about patient safety. Having sufficient or appropriate nurse-to-patient ratio levels is essential to enhancing quality of and accessibility to care. I began the research by using the “Walden Library and Your Total Information Network” to guide me in the necessary steps in locating the scholarly articles. I selected the articles based on my nursing topic and followed the link to the nursing databases area in the library. I saw the list of available databases and I chose on that includes full text articles from the list of “Best Bets”. I chose the CINAHL (Cumulative Index to Nursing and Allied Health Literature) and typed by topic or subject, nurse-to-patient ratios and nursing levels in hospitals and effects on patient care (Walden University Library, 2022). The database brought me to a result page of articles to select and I chose the above article.
Searching for the articles based on the above steps did not present any significant difficulties. The only notable challenge or difficulty was ensuring that I use the specific terms and topics to filter the results and searches for better optimization. CINAHL would be a useful database to my colleagues because it is easy to navigate and filters outcomes appropriately (Walden University Library, 2022b). Further, it provides full text articles with full citations that makes it easy for colleagues to use it in getting appropriate research evidence for their projects. I would recommend this database because of the benefits that it offers like fast and easy navigation of the different components that it has for nurses and other health care professionals and nursing students.

I am currently pursuing the family nurse practitioner program at Walden University. An area that interests me is endocrinology in the primary care setting. I feel that many patients are choosing to have their primary care team oversee their diabetes management as opposed to going to a specialist. It can become very stressful for patients to have to see many different doctors and in certain cases, it is completely necessary to see a specialist. Still, I do believe there are certain diseases that primary care should be able to treat such as Type 2 diabetes adequately. I began my search on Walden’s Library page. Walden’s Library page offers many different resources and databases.
I started first by using CINAHL & Medline combined search, “CINAHL stands for Cumulative Index to Nursing & Allied Health Literature, and includes peer-reviewed articles, reports of evidence-based practice, case studies, systemic reviews, and more.” (Walden, n.d). Upon entering this site, I used two terms, endocrinology and primary care. I came across a peer-reviewed article called An endocrinologist-supported intervention aimed at providers improve diabetes management in primary care site: improving primary care of African Americans with diabetes which concluded by explaining, “Feedback on performance aimed at overcoming clinical inertia and giving to internal medicine resident primary care providers improves glycemic control. Partnering generalists with diabetes specialists may be important to enhance diabetes management in other primary care settings.” (Phillips, et al, 2005). As a focus in my studies, I want to overcome clinical inertia and put ADA standards into practice, and feel comfortable with adjusting PO and insulin regimens for optimal glycemic control in the primary care setting.
Difficulties I experienced while searching for an article were upon typing in my search over 4,000 articles come up, I don’t think it is possible or realistic to be able to shift through every single article. It took me some time to choose an article that spoke to what I was researching. Many of the articles were abstract instead of being able to access full text.
I believe this database would be extremely useful for my colleagues as we are all in these graduate studies programs that require access to scholarly articles and evidence-based practice to apply that research in our practice. The online article Simplifying Synthesis states, “Quality evidence-based practice initiatives and nursing care are based on understanding and evaluating the resources and research available, identifying gaps, and building a strong foundation for future work.” (Shellenbarger, 2016). Understanding and evaluating resources and research available is vital for professional growth so I would definitely recommend this database as I felt that it provides access to quality evidence-based practice while being easy to navigate.

Using the Walden Library
I want to be a nurse who helps various people of all genders, ages, and races. That’s why I am working to become a Family Nurse Practitioner. As a Family Nurse Practitioner, I would have a diverse amount of patients, and I’d be able to help more types of people, whether young or old. Compared to a normal Nurse Practitioner being a Family Nurse Practitioner would be more flexible and enable me to help all people and not just a select amount. To better be an FNP, I need to learn more about the field and also about others’ experiences within this field.
I searched for an article about Family Nurse Practitioners. I wanted to find an article that would help me later on when I become one, giving me good advice and insight I may need later on. This database would be useful to my colleagues who are also interested in practicing in this field, as it gives clinical experiences to other students. By having access to this information, new family nurse practitioners would be ready to face any difficulties or issues that may come their way.
I would recommend this to my colleagues since it gives useful information that would be imperative to their learning and help speed up their process. I had no difficulties searching for this article as the Walden Library found out to be quick and easy to use. All I had to do was search Family Nurse Practitioner, and multiple choices were shown. But, if I were to say something that may be difficult for others, it might be that signing in is required, so it’s not accessible to everyone. Other than that, the site is very user-friendly, and it has a varying amount of resources that would be helpful to anyone.

This article discussed a transitional care model led by a social worker. The Bridge Model is a person-centered, outcome-oriented, interdisciplinary transitional care intervention that helps older adults’ transition from the hospital to their homes and communities. This peer-reviewed article is of particular interest because it explored a different approach to facilitate transitional care. The model produces tangible benefits for individuals and families, pointing toward a future of patient-centered health management. As a nurse, I am familiar with models and interventions led by nurse practitioners; however, the Bridge Model is led by a Bridge Care Coordinator (BCC) and focuses on psychosocial issues that affect older adults during transitional care (Alvarez et al., 2016).
Experience Searching for the Article
I used Walden University (WU) library to search for the article. I entered the keywords in the search box, and I was directed to the EBSCO Discovery Service database that included the link for the item. I did not face any difficulties; the overall experience was easy. WU’s databases contain a variety of materials. Some databases contain only peer-reviewed journals; others include a mix of peer-reviewed and general material (Walden University, 2020a).
Overall, these databases are beneficial during the literature search. Depending on the type of article, students can initiate the search specifying the type of publication. For instance, articles can be located by keywords, title, journal, or Digital Object Identifier (DOI). In addition, the library offers a great variety of dissertations and thesis, newspapers, magazines, encyclopedias, among other relevant materials (Walden University, 2020b).
Usefulness of the Walden University Library
There is no doubt that the WU library is an excellent tool for students enrolled in all types of programs. The accessibility to any material and the library’s easy-to-navigate feature makes it indispensable during assignments preparation. One of my favorite tools is the Ask A Librarian. I have used it on several occasions, and the response is almost instantaneous. When in doubt, I ask questions about references format or in-text citations.
As students enrolled at WU, we have automatic access to a countless number of publications. Getting familiar with the library might be difficult at first, especially if we have a preference for Google Scholar or any other type of database; however, there is a great variety of short tutorials that helps to understand the dynamics of the site. “Library research skills are vital to academic success as students at WU” (Walden University, 2020c); hence, I strongly recommend becoming familiar with it. Let us not forget that most assignments require the use of the WU library.

I’ve discovered that search terms, or keywords, are crucial. Keywords are search terms that you use to describe each of your concepts in the database. When you search the database, you are usually looking for words in the Title and Abstract rather than the full-text article. The author of the article creates the title and abstract. The database will attempt to match your keywords against the author’s words in the title and abstract, returning only results that match what you enter.
All of this is to say that databases frequently look for the exact words and phrases you type in, so if the author uses a different word (synonym) to describe a concept, you will not see that article in your results. To solve this problem, you should identify alternative keywords for each of your concepts. You can do this by asking yourself, “What other words could the author use to describe this concept? Use caution when using phrases. When searching with a phrase, consider other ways to describe the phrase and search with those as well. For instance, hand washing, handwashing, or hand hygiene. If the subject is unfamiliar to you or you’re having trouble coming up with synonyms. For background information, you can consult dictionaries, textbooks, or encyclopedias.
Finally, make a master list of synonyms for each of your concepts. Use this list when searching the databases. Be creative and consider: Related Words/Spelling Variations (especially American vs. British, for example, anesthesia or anesthesia/ Acronyms (also spell out the phrase) /Brand and generic drug names/ Plural and Singular Variations/ Narrower Terms/ Broader Terms. These can all be helpful steps in locating the exact information you’re searching and in improving the quality of your research.

Engaging in scholarly writing is a significant component of being successful in a graduate program. “Scholarly writing is material written for a specific purpose and for a specific audience that is based on evidence, not assumptions or opinions” (Laureate Education, 2018a). While most individuals are familiar with professional writing, which is based on experience, not many are accustomed to scholarly writing, which is always evidence-based, clear, concise, and objective (Laureate Education, 2018b). In the master’s program at Walden University we are expected to participate in scholarly writing by using credible sources to guide our writing and ideas (Laureate Education, 2018a). Credible sources may be difficult to identify amongst the vast array of information that is at our fingertips on the internet. Luckily, there are many databases available to assist in locating credible, scholarly, evidence-based information to lead us on the path to efficient academic writing.
One example that is easily accessible to us as students at Walden University is the Walden Library. Walden’s library is an online database that includes a “searchable digital collection of different types of materials such as books, journals, magazines, and newspapers” (Walden University, n.d.). Through the Walden Library, we can locate thousands of scholarly, credible sources and obtain reliable information without deciphering between objective and subjective material, as we would if we used non-scholarly search engines, like Google. For example, when searching for a credible, scholarly article regarding nurse practitioners as primary care providers on both Google and the Walden library, I found a significant difference in the search results. When I searched “nurse practitioners as primary care providers” in Google, it resulted in several different organizations explaining the increasing need and expanding role for nurse practitioners as primary care providers. Although some of these organizations were credible and reliable, the information did not seem substantial and objective enough to incorporate into a graduate-level scholarly piece. On the other hand, when I searched the exact phrase into the Walden student library, it led me to Ebscohost, which populated hundreds of scholarly, credible, objective articles regarding nurse practitioners as primary care providers. Ebscohost is an online database that “offers high-quality articles licensed from reputable publishers recognized by library professionals, chosen to meet the specific needs of researchers” (Ebsco Information Services, Inc., n.d.).
Through the Walden Library and Ebscohost, I was able to locate an academic journal article by Miner (2019) called Physicians, Nurse Practitioners, and the Shortage in Primary Care Providers: Professional Autonomy in the Public Discourse. This article sparked my interest because it is a peer-reviewed, scholarly journal article with factual, objective information that thoroughly outlines the vital role that nurse practitioners play in society with the ongoing primary care provider shortage (Miner, 2019). While the journal article was easy to find once I searched for the appropriate keywords, I did encounter one minor difficulty that mainly involved user error on my behalf. When I first logged on to the Walden library, I searched all in Thoreau for the topic of “nurse practitioner.” This search did not retrieve the information I was looking for because I did not make the search criteria specific enough. Once I modified the search to “nurse practitioners as primary care providers,” I found an abundance of relevant information regarding the subject that I had chosen. The lessons learned were to always be specific with the search criteria and use keywords to yield the best database results for scholarly sources.
Given the ease of using the Walden Library to find hundreds of articles on Ebscohost, this database would be extremely useful to my colleagues, and I would undoubtedly recommend it. Using Ebscohost was not only simple to maneuver, but it also generated an extensive search of thousands of credible, evidence-based, objective articles that are critical to achieving scholarly writing at a graduate level. This information would be advantageous to my colleagues because it would allow them to incorporate dependable sources through an effortless, simple-to-use database. Ultimately, “successful scholarly writing requires researching and presenting information objectively to an academic audience,” and the best way to accomplish this is to start by incorporating evidence-based, objective information from credible sources through the use of databases like Ebscohost found through the Walden Library (Laureate Education, 2018a).

I am currently trying to decide the specialty track I want to complete. Mental health and family practice are two great areas, but if I had to narrow it down to one, I will probably choose Mental health due to the increasing demand for care in this area. Being able to acquire up to date data that is focused on evidence base practice is especially important. The database I utilized was the TRIP database located on the Walden Library site. This database contains evidence-based responses which its authors are both peer and non-peer reviewed. Once you search your topic, it gives you the opportunity to utilize filters to narrow the search of what you want. I think this resource would be greatly used by my colleagues because it gives them the opportunity to search for topics or data with ease and over a wide span from blogs to videos, to pamphlets and viewpoints from all over the world. The only difficulty that I encountered in the beginning is understanding the filter options but once you maneuver through, it was easy to find exactly what you were looking for.
The article I chose speaks on the positive role that practitioners play when dealing with veterans. According to York et al. 2016, this article deals with the Veteran centric domain, which is a tool that is developed from literature on knowledge, attitudes, and skill behavior to assist patients in dealing or coping with mental disorders. This program extends to the families of these veterans to assist them in the care of the veteran as well. With more innovative components, such as this practice, developing in order to meet the demands for training practitioners to better understand mental health, I believe that we will be able to deliver exceptional care in the years to come.

I utterly understand how you feel in trying to decide which specialty you want to complete. I believe it is quite normal and something worth thinking about. I decided to pursue my Master of Science in Nursing psychiatric mental health nurse practitioner (PMHNP). However, I sometimes wonder if I am making the right decision even though I am working in the mental health department for over three years. We are often faced with patients experiencing “agitation, an acute behavioral emergency requiring immediate intervention” (Richmond et al., 2012). Whether through verbal redirection, pharmacotherapy, mechanical, or physical restraints to the point of de-escalation. The safety of the patients and staff is paramount.
According to Kverno and Mangano (2021), overcrowding in the Emergency departments (EDs) are increasing, especially during the coronavirus pandemic. Psychiatric patients experiencing psychosis must be evaluated medically and screened for risks of harm to self or others before they can be discharged or transferred to inpatient units. PMHNPs in the psychiatric emergency department (ED) are often the first contact for assessing, diagnosing, and prescribing treatment. With, that being said, there is a need for health care professionals for all the specialties, and we have our duty to perform. On the other hand, we are all human, therefore being in the specialty of your calling will offer a feeling of gratification when the care you provided changes someone’s life in a positive way.

I agree with you. Mental health is currently on high demand and provides a good experience in health care. In the contemporary world there has been an increase in the cases of mental health issues which coincides with the pandemic and general new world issues such as technology. The database chosen is very useful and provides articles on different relevant topics. Using peer reviewed articles in doing research provides insight that are well backed by science and approved. When using the databases for the first time it might be difficult, however, makes work easier being that one can choose certain filters that narrow down the search to what someone wants.
