A nursing practice issue is how a person responds to existing or potential health issues (NANDA International, n.d.). Teaching a patient who gets a deep vein thrombosis how to self-administer Lovenox is an illustration of a nursing practice difficulty. A medical practice issue focuses more on the illness or the pathology causing the illness (NANDA International, n.d.). An illustration in this situation would be determining whether anticoagulant therapy would be best for the patient after diagnosing deep vein thrombosis. The impact that the medical issue has on the patient is the basis for the nursing issue.

Evidence-based practice is the bedrock of nursing care (EBP). When EBP is used to provide patients with safe, high-quality care, patient outcomes improve. It is critical to formulate the PICOT inquiry with background and foreground questions to ensure that PICOT is based on a nursing practice problem (Stillwell, Fineout-Overholt, et al., 2010). PICOT is used to collect information about the patient population, the intervention or issue of interest, comparisons of the intervention or issue of interest, the desired results, and the time required for the intervention to produce the desired outcomes (Stillwell, Fineout-Overholt, et al., 2010). This evidence supports the study’s necessity. Because it is based on a problem in nursing practice, the PICOT material will include the best data from prior research to assess improvements in treatments and education.

Conduct a literature search to locate research articles focused on a practice problem of interest. This literature search should include both quantitative and qualitative peer research articles to support your practice problem or issue of interest in 350-750 words.

Identify six peer research articles which will be utilized through the next 5 weeks as reference sources. Create a reference list in which the six articles are listed. Beneath each reference include the article’s abstract. The completed assignment should have a title page and a reference list with abstracts.

Suggestions for locating qualitative and quantitative research articles from credible sources:

Use a library database such as CINAHL Complete for your search.

Using the advanced search page check the box beside “Research Article” in the “Limit Your Results” section.

When setting up the search you can type your topic in the top box, then add quantitative or qualitative as a search term in one of the lower boxes. Research articles often are described as qualitative or quantitative.

Topic 1 DQ 2

Nursing research is used to study a dilemma or a problem in nursing. Examine a problem you have seen in nursing. Provide an overview of the problem and discuss how addressing the problem through nursing research can improve patient outcomes. Provide rationale and support for your answer.

Dilemma is often an issue that appears in nursing. Nurses are place in a sometime difficult and uncompromising position. From giving advice to a patient about their health or decision regarding care. (Dilemmas in nursing, 2022) Example: a patient refused having an x-ray done at bedside due to chronic pain. Patient has several decubitus and because the film will need to be placed underneath the patient back it will cause more pain and pressure to the patient. As a nurse we must explain the reason and benefit of having the x-ray done. Patients have a right to refuse treatment or any procedures. Although the patient refuses the x-ray the patient knew that continuing treatment was important regardless of having the diagnostic test. Patient did understand the importance of the test but is also aware that for the providers to further treat the disease or disorder a diagnostic test need to be done.

To address the issues or dilemmas that the patients maybe experiencing is not to have any judgement but to understand their concern. This can be done be letting the patient know that they are being heard. As nurses we need to help figure out an alternative method of helping the patient with their fear and pain to eventually move forward with getting an x-ray done. The patient has lack of control of most treatments done in the hospitals, the question to ask is what can be done to gain more control in the hospital. It will help the patient achieve a better outcome if you negotiate and express what you and the patient both cares about. (Robert A., 2015)

Topic 1 DQ 1

The distinction between a nursing and a medical problem stems from the nature of the assessment and application of the research intervention. Imperatively, a nursing practice is one that occurs and is detectable during nursing assessments based on the condition or disease of a patient. A nursing practice problem includes patient care needs from the moment they enter a healthcare setting, during a physician diagnosis, and after care provision evaluation. The implication is that a nursing practice problem assessment requires a collection of comprehensive data on a patient in order for a nurse to provide appropriate interventions and achieve appropriate outcomes within a specific time frame of care (Grand Canyon University, 2018).

A medical practice problem, on the other hand, arises from a medical condition in which a physician needs to develop interventions to assist them in diagnosing a condition. For example, a medical practice problem for a cancer misdiagnosis may arise because an oncologist may order different diagnostic tests instead of the required ones. A nursing practice problem based on a stroke diagnosis, on the other hand, necessitates a nurse having information on the patient’s pathology as well as risks for other conditions such as falls and a more holistic understanding of the effect of stroke on the specific patient as well as family.

In practice, the PICOT framework allows nurses to identify a population problem that requires effective intervention. The implication is that PICOT should be based on a nursing practice problem in order for nurses to understand the population and formulate effective interventions in comparison to others in order to deliver quality care outcomes (Melnyk et al., 2016). Patients who have had a stroke, for example, are more prone to falling. As a result, falls in patients are a problem that nurses must address in order to improve patient satisfaction.

PICOT is utilized by the health care community to identify and study a nursing or medical practice problem. Consequently, PICOT examples that may provide insight into the use of the PICOT process, may not be relevant to nursing practice as they are based on a medical practice problem.

Describe the difference between a nursing practice problem and a medical practice problem. Provide one example of each. Discuss why is it important to ensure your PICOT is based on a nursing practice problem.
