Cohesive Collaboration is important to the survival of and success of an organization. Implementation plans can face numerous challenges as a result of lack of knowledge of the problems hurting an organization. This lack of knowledge is common in many organizations were leaders and management are disconnected from day to day operations that help to run the organization. When a person rises through the ranks of the organization into a leadership role they have knowledge of and experience that other managers lack if they hired from the outside. The person who rises through the ranks has more understanding of the how the organization works, the weakness that exist, the organizations strength and how the organization needs to function in order to survive. Where there is s disconnect between stakeholders in higher positions and those working the day to day operations, noting will get done.
Some of the things that one might experience during the implementation phase are resistance and timing. “Lack of time is often cited as a barrier to implementing EBP. In addition, uncertainty or lack of knowledge about the EBP process is also a barrier, which includes critiquing and appraising the literature related to the clinical problem being addressed” (Ginex, 2018). In busy hospitals lack of time to study and understand new changes and policies can be a barrier. On time of this, resistance to change can also be a barrier. In there is already a flow that nurses are accustomed to even if it is chaotic, having to change that flow requires learning something new or changing ones workflow habits and that can cause resistance to change.
The one suggestion I would give to a fellow colleague would be for them to ask questions, slow down the pace in order for them to understand the new policy and give themselves time to become accustom to it and to always remember that it will take them time to learn something new before they can master it.

Stakeholders are the people involved in carrying out the process under review or individuals who become affected with the change in practice (“John Hopkins Nursing”, 2021). It is important to determine the potential stakeholder barriers that will be encountered in order to address them accordingly. The two stakeholder barriers that can be experienced are resistance to change and lack of authority. Physicians, who are one of the main stakeholders, are perceived to be the least supportive in EBP implementation (Jordan et al., 2021).
Without their support, it can make the implementation process difficult. Resistance of practitioners to change from traditional practices to newer concepts introduced in EBP can be a barrier to the implementation process (Jordan et al., 2021). Moreover, some staff nurses feel that they lack authority to make changes in their unit or they do not have the “power” to implement the EBP project. One study suggested that majority of the nurses in a private ICU reported that they lacked the authority to change practice in order to facilitate the implementation of EBP (Jordan et al., 2021).
There are several strategies to gain stakeholder support. First, the support of physicians for the implementation of EBP should be encouraged (Jordan et al., 2021). It is important to keep an open communication between the researcher and physicians. Explaining the goals and benefits of the EBP project in improving patient care can help gain their support. Second, it is recommended to carefully identify individual barriers versus organizational barriers at an early stage (Jordan et al., 2021). Lastly, identifying your stakeholders as well as engaging them early in the EBP process is important because stakeholder buy-in is important for its successful implementation (“John Hopkins Nursing”, 2021). The goal is to gain their trust and confidence and gain as much support as possible for the successful project implementation.

As an evidence-based practice project implementation leader, some common stakeholder barriers that may be encountered include resistance to change and lack of awareness of the benefits of the project. These barriers can significantly hinder project success and delay implementation. Resistance to change is a common barrier that arises when individuals feel that their routines or processes will be disrupted. This may occur when staff members are comfortable with their current practices and do not want to deviate from them (Crawford et al., 2022).
To gain stakeholder support, it is crucial to communicate the importance of the project, highlighting the benefits and how the project will ultimately improve patient care. Educating staff members on the importance of the project, its relevance to their daily practice, and providing them with the resources they need to implement it can also be effective. Another barrier is a lack of awareness of the benefits of the project, which may be due to a lack of understanding or communication. To address this, it is necessary to engage with stakeholders through meetings and presentations to communicate the project’s goals and objectives. Additionally, involving stakeholders in the planning process and providing opportunities for feedback can help to address any concerns they may have.
If a colleague is struggling to gain stakeholder support, a suggestion would be to collaborate with influential stakeholders who support the project’s implementation and form a coalition or team. This coalition can serve as advocates for the project, helping to address any concerns or misunderstandings that may arise. In addition, seeking the support of opinion leaders and key influencers can help to build momentum and increase stakeholder buy-in (Mejia et al., 2021).

Two stakeholder barriers that I may experience during the implementation phase of my evidence-based practice (EBP) project include leader/manager resistance and lack of belief that EBP will result in more positive patient outcomes (MeInyk & Fineout-Overholt, 2019). The nurse manager and director of nursing stakeholders at Crouse Hospital may be resistant to implementing an EBP project in the hospital during a staffing crisis. These stakeholders may feel that time and financial resources should be used for other initiatives, rather than my EBP project. One strategy to increase support from these stakeholders is to show them evidence from current research that shows that a decrease in delirium is both cost-effective and saves clinical time (Mathieson et al., 2019).
Staff nurse stakeholders may have a lack of belief that EBP will result in more positive patient outcomes. Nurses who work in the ICU do not always see the long-term negative impacts of delirium, as they only treat the patient while they are acutely ill. Using the strategy of training nurses on the innovation before it is adopted could help to increase staff nurse stakeholder support (Mathieson et al., 2019). Part of the training could include stories from patients who have had negative long-term health complications due to the development of delirium in the ICU. This education could help staff nurses better understand the potential for improvement in patient outcomes through implementation of this EBP project.
One piece of advice I would give to a colleague who is struggling with obtaining stakeholder support would be to find out what is important to each stakeholder and what their goals are for improving nursing practice and patient care. Once they have gathered this information, they can present their EBP project in a way that addresses each of the stakeholder’s priorities and goals. By showing each stakeholder that the EBP project complements the work they are already doing, the stakeholders will be more likely to support the project.

Stakeholders are a major part of the project. We need to make sure they are involved 100% and for the right reason. I believe that the stakeholders will be people from various backgrounds, both clinical, educational, and lay people. One barrier could be that since they do not share the same background that they may not fully understand or support the project. According to this article, what we can expect from stakeholders is stated as, “The innovation of Theory of Change lies (1) in making the distinction between desired and actual outcomes, and (2) in requiring stakeholders to model their desired outcomes in an order in which they need to be achieved before they decide on activities, tactics, strategies, and other forms of intervention (Theory of Change in a Nutshell, 2021). There is a good deal of discussion as to which provides more value to practitioners and stakeholders—the group process of reflecting on the work, surfacing assumptions, creating transparency and building consensus; or the product, a sound and complete plan with plausible potential for producing the change desired.
(Theory of Change in a Nutshell, 2021) If I were giving suggestions to a colleague struggling with obtaining stakeholder support, I would begin by looking into the heart of the situation. The moral concept of what a stakeholder is and can be expected to bring to the table. We as human beings need to be exactly what we expect others to be for us. This article reveals this moral concept. This moral concept of stakeholder love offers promising contributions to stakeholder theory, leadership theories, and ethical theories in general and business ethics theories in particular. (Kaptein, 2022) The adoption of this duty by workers could also have positive implications for themselves and their neighbors.
This article establishes that there is a secular moral duty of neighborly love in one’s work by first examining whether there is a secular approach that sufficiently grounds a moral duty to love either one’s neighbors in general or one’s neighbors in the context of one’s work. (Kaptein, 2022) However, Kant does not reject the duty of neighborly love. He argues that there is a moral duty of practical love, which he defines in terms of benevolence or doing good to others. (Kaptein, 2022) If we hold this moral concept as our standard, we should then have a solid foundation in implementing an evidence-based project that will be successful.

One stakeholder barrier I may encounter during the implementation phase of my evidence-based project is provider support. Providers are a key stakeholder in my project as they will be responsible for further assessing patients and providing resources as needed. There is often a barrier when there is hierarchy within an organization, most notably among physicians and providers (Pitsillidou et al., 2021). Another stakeholder barrier I may encounter revolves around the stigma surrounding mental health and the lack of funding for mental health globally (Murphy et al., 2021). The perception surrounding mental health may influence providers who believe it is not within their specialty to provide a mental screening. Moreover, patients and families may not be willing to undergo a screening related to mental health due to societal stigma and misconceptions.
A strategy to gaining stakeholder support is ensuring that clinical management is in support of the project. These managers will have a bigger say in limitations related to costs but will also appreciate the improved care that the project provides to patients (Quanbeck, 2019). Additionally, during interactions with patients and their families, it is important to provide education regarding mental health and the fact that pediatric patients with type 1 diabetes are at higher risk for disorders such as depression, anxiety, and suicidal ideation.
If a colleague approached me because they were struggling with obtaining stakeholder support, I would recommend that they compile literature regarding their subject. With this in hand, I would recommend they set up a meeting with their clinical manager to discuss their evidence-based project, the research to support their project, and their goal to improve patient safety and healthcare delivery.
