I followed the recommendation of the Translational Medical Research guide in the Nursing and Health Sciences GCU library and focused my search on the geriatric population. I found an article in the Journal of the American Geriatrics Society via PubMed related to frailty. This topic caught my attention because this is often discussed when we evaluate a geriatric patient for transplant candidacy. In my observation in the clinical setting, frailty is often determined by the physician based on the patient’s BMI, a sit-to-stand test to measure strength and endurance, and whether the patient is healthier than others of similar age. The evaluation is not done with a frailty assessment tool.
As stated by Walston et al. (2019), the assessment of frailty is frequently done on older adults needing to undergo invasive medical or surgical procedures and can effectively identify patients with increased risk for adverse health outcomes. Frailty research has been on the rise in the past 20 years and is being practiced in the United Kingdom with interventions using nutritional and exercise strategies, but this is not a routine clinical practice in the United States and many other countries in the world (Walston et al., 2019). In relation to the phases of translational research, this is in phase T3, also known as dissemination research. It focuses on “moving evidence-based guidelines into health practice” (Institute of Translational Health Sciences, n.d.). The article clearly states the intention of accelerating the integration of frailty assessment into clinical practice in the United States. A symposium was organized in September 2017 for the purpose of sharing information on this topic. They emphasized major barriers, pointed out the lack of clinical and pharmacological treatment plans despite the identification of this condition, and the development of the next generation of frailty research focused on clinical practice integration.
