According to the consensus report of the National Academies of Sciences, Engineering, and Medicine, The Future of Nursing 2020-2030: Charting a Path to Achieve Health Equity, “The authoring committee produced a set of recommendations for the field of nursing and it provided a blueprint for improving nurse education; ensuring that nurses can practice to the full extent of their education and training; providing opportunities for nurses to assume leadership positions; and improving data collection for policymaking and workforce planning.” Out of the nine key recommendations the consensus report has addressed, the two main ones that I consider to be on the top level of importance within my community and organizations involve the nurse’s health and wellbeing and the inequities surrounding public health emergencies.

Recommendation 8 hits close to home on so many levels. I’ve been a nurse for three years, and the pandemic surprised me. I had been a nurse for about a year when Covid began. The amount of stress and distrust that my coworkers and I experienced while navigating the ever-changing policies and procedures associated with the pandemics CDC instruction was unnecessary. Unfortunately, I witnessed our organization’s flagrant disregard for our health and well-being by withholding and rationing critical supplies that were easily accessible to us. It was difficult at the time because it was a whole new ballgame for even the most experienced nurses, and a significant number of employees resigned as a result. We felt abandoned because management never showed up on the unit like they used to. Bringing our concerns to our director and management during staff meetings was futile, as CDC recommendations and the organization’s rationales based on fear and ignorance would prevail. The COVID pandemic served as a wake-up call to the world in terms of high-level planning and preparation for patient and provider safety.

The Institute of Medicine (IOM) proposes that, in order for the healthcare industry to satisfy the increased demand for quality and economical treatment, the nurse training framework be redefined and modified to align with changing healthcare trends. Expanding the nursing faculty and improving the capacity of the available nursing schools are among the recommendations for nursing education to boost effectiveness. Furthermore, there is a need to reframe the nurse training program in order to broaden the scope of training and ensure that nurses graduating with a BSN are well trained in the incorporation of technology, research methods, basic statistical data analysis and interpretation, and most importantly, they are well equipped to provide education to patients and emphasize the current role of the nursing profession. According to the ACA, nurses are not only expected to provide interventional measures in terms of treatment and patient care, but they should also prioritize preventive measures in their care services. It is critical that nurse training institutions assist student nurses in honing their communication skills and instilling some pedagogical skills in order to enable them transmit the message of health promotion measures as a means of enhancing healthcare and promoting quality of life (Cleveland et al., 2019). I agree with the IOM on the changing role of advanced nursing practice. The recommendations remove limitations on the APRN’s scope of practice and allow them to practice to the full extent of their education and training (Salmond & Echevarria, 2017).

Who regulates APRN scope of practice?

The regulatory mandate for the APRN practice is regulated by the state’s boards of nursing. Each state has its own regulatory laws that govern the scope of practice of the APRNs (Staff Writers, 2019). In some states the full practice is allowed while some they have the limited practice regulation. The full practice regulation also has three other categories. There are those states that allow full practice without the transition period, some allow collaboration between the NP and the APRN, while some allow collaboration with physicians only. Likewise, the restrictive practice regulation has two folds, those that restrict only the authority to prescribe while other states have more than the prescriptive restrictions (STATE LAW FACT SHEET: A SUMMARY OF NURSE PRACTITIONER SCOPE OF PRACTICE LAWS, IN EFFECT APRIL 2016, n.d.) In the Institute of Medicine (IOM) report title “The Future of Nursing: Leading Change, Advancing Health” (2011), the IOM recommended changes to the nursing profession in order to improve patient outcomes. One of those recommendations is to “ensure that nurses can practice to the full extent of their education and training” (Institute of Medicine, 2011). These is probably the most important aspect of the IOM report because although we are facing a shortage of primary care providers in the country, many regulatory bodies still put boundaries in place for nurses trying live up to this recommendation. There are currently only 20 states in America that allow Nurse Practitioners to practice without a supervising physician (Full Beaker, Inc., 2021).
Another recommendation from the IOM report is that nurses should achieve higher levels of education (IOM, 2011). While I agree wholeheartedly with this recommendation on the surface, I believe that it may help and hurt the profession. Nurses should be expected to achieve higher levels of education and continue their education throughout their career because its just the right thing to do. Health care is changing constantly and nurses should keep up with those changes in order to keep patients safe. However, if the minimum educational level for entry into the nursing profession were to be changed to a baccalaureate degree it would prevent many people from entering the profession in the first place. The time and cost involved may be a deal breaker for many. In addition, with the nursing shortage getting worse every year we should not increase the time it takes nurses to obtain licensure. I know that for me, if I had to go straight through for a bachelor’s of science in nursing, rather than obtaining my associates degree, I would probably have chosen a different profession.

I do agree with you that one of the best recommendations by IOM about the role of the advanced registered nurse practice is to allow the nurses practice to the full extend according to their level of training and education. This has a direct impact on the effect of shortage of primary care providers in the sense that it will relieve the pressure on the already outstretched and overwhelmed primary healthcare providers. Additionally, the same concept was emphasized by the Center for Medicare and Medicaid Services (CMS) which broadened the concept of “medica staff” (Altman et al., 2016). This is meant to ensure that all healthcare facilities allow other healthcare practitioners like the APRNs to perform a number of roles that is within their area of training, practice and experience. Healthcare organizations that apply these recommendations are bound to be effective in terms of offering timely primary care for the patients and ease the pressure on the physicians. Additionally, this will encourage multidisciplinary health collaborations that are likely to result to high quality services and improved patient outcomes (Leach et al., 2017).

The text The Future of Nursing: Leading Change, Advancing Health contains a few recommendations for nursing education that are thought to bring about positive change in the nursing field. One of the recommendations that I believe will be radical in bringing about change in the industry is to increase the number of baccalaureate-degreed nurses by 80% by 2020. Despite the fact that 2020 has passed, there is an increasing demand for nurses who are skilled and knowledgeable enough to meet the health care system’s ever-increasing challenges. This recommendation necessitates the collaboration of public, private, and organizational structures to provide scholarships, funding, and loan forgiveness for nursing education. Employees should encourage workers with associate degrees to return to school and earn their baccalaureate degrees in exchange for tuition reimbursement and promotions or salary increases at work. This arrangement has benefited me. My company provides some sort of scholarship for further education. The importance of diversity in race/ethnicity, gender, and geographical distribution should not be overlooked. This is one solution to the problem of health inequity in the United States. For example, mistrust of white health personnel prevents Black people from seeking medical attention and instead self-medicate. The availability of diversity would increase the accessibility of health care to diverse populations.
The second recommendation that I strongly support is that nurses participate in lifelong learning. If the faculty works with health systems to update their curriculum and competencies, they will be better prepared to teach the next generation in an ever-changing health care system. Similarly, student nurses graduate from colleges better prepared to face real-world challenges. This saves health care systems a lot of money and time in orientations. When I first started working as a nurse, I believe I would have been better prepared if it had been taught in school. Although I learned on the job, schooling would have better prepared me.
I agree with the IOM’s assessment of the changing role of the Advanced Practice Registered Nurse. It described the changing challenges of health care, such as chronic conditions, an aging population, a more diverse population, health disparities, and limited English proficiency, and how APRNs have been trained to meet these changing needs through seamlessly coordinated care, coaching, and innovation.

I do agree with you that one of the recommendations to bring about a radical change in the field of nursing is to all public and private partners to collectively help the practicing nurse to continue with their education and training and at least have a minimum qualification of a baccalaureate degree. This can be achieved by healthcare organizations to have programs of tuition fee reimbursement for its staff. Even though healthcare organizations could be facing serious financial crisis, it is imperative for this organization to work on cost effective means promote continuous staff development and advancement as an investment to help improve on quality of care and hence attract more customers (Stadhouders et al., 2019). I do agree with you that the process of continuous and lifelong education is important to help the nurses adapt to the rapidly changing healthcare landscape and be able to adopt the emerging technologies and skills needed to offer quality care to the patients. Advanced education: Changes that occur in health care has caused changes in nurses’ roles. The involvement of nurses in advanced education allows the use of evidence-based practice to enhance positive patient outcomes (IOM, 2010). Nurses need advanced education because of emerging diseases including advanced cancers which needs intense treatment to achieve good patient outcome. Advanced nursing education also allows advanced practice nurses to practice to the fullest of their training and help to decrease physician shortages and brings cost effectiveness to healthcare (IOM, 2010). My participation in this class is an evidence that I am advancing my education to perform at my fullest capacity as a nurse and catch up with the changes that occur in healthcare today.
Accelerating personal competence: It is very important for nurses to improve their personal competence because new equipment, technology and nursing processes are becoming more available (Weberge, Mangold, Porter-O”Grady, Malloch, 2019). The more a nurse can learn about new approaches the more informed the nurse becomes and the better the nurse can evaluate the effectiveness of the new approach for organization and if it is good. This approach reinforces evidence-based principles as the supportive rationale for change (Masters, 2015). My experience as nurse verifies this required change in nursing. I have benefitted from improving my personal competence by updating my knowledge on new nursing or medical equipment, new technology and changes in nursing practice or approaches. I take some online competency tests and continuous education on courses that can help develop my competency as a nurse.
I agree with the description of Advanced practical registered nurses’(APRNs) role as evolving by IOM because APRN’s are needed to fill the available healthcare providers positions and reduce the shortage of physicians both in rural and urban areas (IOM, 2010). The existence of APRN’s has given many people the opportunity to have access to health prevention and maintenance as well as easy access to health care without waiting for too long to see a healthcare provider (IOM, 2010).

I do agree with you statement that the concept of continuous education among the registered nurse practitioners and the APRNs has increased the capacity and the applicability of the EBP projects utilization in the healthcare industries. Nurses who have a higher level of education have the relevant training and expertise to carry out successful EBP and generate new knowledge and findings on how best to improve on the practice and promote primary care. Apart from being initiators of the EBP projects, such nurses are able to utilize other research findings and easily translate the information into practice hence help bridge the translational gap between EBP results and the nurse practice (Mathieson et al., 2018). I alos agree wit you that the evolving role of the APRNs as recommended by the IOM is the appropriate step to move towards enhancing accessibility to primary care and removing the barriers in the nurse practice can go along way in boosting their morale and helping in the nurse retention rate hence alleviate to some extent the shortage of primary care providers (Altman et al., 2016).

I have been using APA style formatting for my BSN papers, the only different is I use the lower edition of APA style template before, now MSN program are requiring to use 7th edition, I have noticed some different from the formatting like when citation more than three authors, now can be shorter, (last name et al). It is very straight forward for the APA style since it can be downloaded from the GCU website student success center or writing center and you can search APA 7th edition; There are two version of the APA templates, one is with abstract and the other do not. I am so used to use the GCU writing center since my past experience. In the Citing sources in APA 7th edition literally listed the what’s new in APA and guidelines, it is an easy way to formatting your papers. For recommendation are download the 7th edition guidelines and the 7th edition templates on your laptops, use it whenever the class assignment requires for APA style. I have profound knowledge and experience in regards to the application of the APA Style. In all my academic writings during my undergraduate studies, the APA Style was among the requirements. Additionally, I have incorporated this writing in my professional writing. As such, the APA Style is one that I not only understand its basics but also one that I am comfortable and enjoy using.

Although I emphasize on my knowledge and experience of this writing style, I was surprised to learn new guidelines from the resources provided. For instance, I now understand that I am required to abide by the requirements of the APA 7th Edition Style. One particular guideline that I will be applying throughout the duration of this course and in future writing is the proper formatting and the application of the abbreviations such as et al. in in-text citations.

I have had experience with APA Style Guide for formatting assignments when I was working on my BSN over 10 years ago. Since it has been so long, it may take me a moment to feel confident again. I choose this link because it has been so long since I have written papers and it includes a nice overview and easy way to navigate all of the different areas of formatting. I will be referring to it frequently. I really like how it has specific examples of different references. I think this will be the most challenging area to get used to again. A couple items that have changed since I last used APA is only using one space after sentences and using “they” as a singular pronoun. I think it will benefit me to purchase a book to refer to also. Having a tangible book to flip through may be helpful in addition to all of the links provided.

My experience with using APA style is, shall we say, dusty. Having not been in school in over 20 years, I haven’t used APA style in quite some time. I am finding the APA style writing guide available through the GCU Student Success Center to be helpful. I like the way it is laid out and the explanations are clearly communicated. That being said, I plan to review all of the guides offered to increase my comfort level with using APA formatting.

I would say that I am fairly experienced with APA since I used it for all of my papers in my undergraduate program, but it has been quite some time since using it. I remember the basics of APA, but I have been having to use resources to verify that my APA format is correct.
I chose this link because I learn best by looking at specific examples. When it comes to using citations within APA, there are so many different variables that can slightly change how something is cited. Since it has been years since I have used APA format, I will be using this link every time I use in-text citations to verify that my work is done correctly.

What is your experience with using the APA Style Guide for formatting your assignments?
In GCU, the recommended format is APA style. I had no prior experience using style. In fact, having obtained my BSN from a foreign school in 1996, we don’t have strict formatting rules in undergraduate studies. If we must write essays, we just follow simple paragraph rules with minimal focus on citations. So writing assignments using the APA format has been a challenge for me so far. I needed to triple-check my writing before I upload it for submission to ensure that I did not miss any punctuation, upper or lower case letters, or correct citation and reference formats.

I found the citation guidelines in the APA format style most challenging for me to learn. I had to keep going back to my resource page to ensure that the formatting is correct for both in-text citations, and references, as well as the format for different sources. You need to know how to format if your source is an article or journal, a book, a website, a dissertation or report, or other online sources. I expect to be using it every week since most of the class discussions and assignments will require references, therefore citations for these references. I plan to keep reviewing the resources available to ensure that my formatting is accurate and take feedback from the instructors for improvement.

I used the APA style formatting for my BSN program at GCU. I used the basic version of the APA style to format my papers. For my graduate program, I have to use APA 7th edition, which has some differences from the basic one. The main difference is in citations.

The future of nursing we’ll see transformations in all aspect of how nurses perform their duties, expectations in their level of education and skill set, as well as increase visibility within our society and in communities. The pandemic nearly upended our healthcare system. As a nation, we saw how vital the nursing profession played in helping to save the lives of people from the COVID-19 outbreak and how much nurses are actually utilized and underappreciated. We also saw how far nurses are capable of achieving with the right guidance, training, education and resources to provide health care services to a growing population and a population that is vastly underserved within out nation.
The pandemic uncovered how underutilized nurses are and given the right support, they can be further used to cover many areas of public health needs. The unfortunate thing about the state of nursing now is the massive shortage that continues to get worse, leaving the public extremely vulnerable. As a nurse of over 30 years, I have watched nurses, achieve great accomplishments and watched as more and more nurses seek higher advancement, education, skillset, visibility and demand a seat at the table when it comes to decision making in public health and patient care policies. I agree in regards to many aspects of how the report describes the evolution of the advanced practiced registered nurses role. The nursing profession is the largest profession in the healthcare field The profession can be utilized in ways such as community outreach on a wider scale to reach vulnerable populations that have difficulty accessing health care services and resources. They can be used for health education such as disease prevention, assisting with early screening of disease, informing the population treatments, disease prevention. Nurses need to be utuilized more as we continue to deal with a population with growing and complex health care needs. “The training and competency-development needed to prepare nurses, including advanced practice nurses, to work outside of acute care settings and to lead efforts to build a culture of health and health equity, and the extent to which current curriculum meets these needs” (National Academies Press, 2021).

The health and well-being of our nation should be considered a priority if we want a strong economy. The challenge is now on the nursing workforce engagement to achieve higher education to address the increasing health care needs of our aging population, poverty, immigration issues, physician shortages, mental health conditions, high maternal mortality rates, and other healthcare-related imbalances. The nurses are being thrust into advanced practice roles but there are limiting factors that need to be eliminated to be fully functional in achieving health equity that will not just boost the health of the community but the nation as a whole. The Committee framework identified the areas to be strengthened to meet all challenges and that includes nursing workforce, leadership, nursing education, well-being and emergency preparedness and response, and structural and individual determinants of health.
Lifting state-level regulations which involved restricting the scope of practice of APRNs is an initial step in recognizing the advanced level of education of nurses and their ability to provide high-quality health care services. Another recommendation is better payment models. I don’t have good knowledge about the current payment system. According to the Academies, the current payment system doesn’t take into consideration the payment for services that address social needs and social determinants of health or advance health equity. These need to be improved further so we can be assured of achieving of health equity. As nurses take the advanced practice roles, they become an effective health care providers and as a result social and economic improvement is also be achieved.
Another recommendation is to strengthen nursing education. Nursing coursework and learning hours should prepare nurses to promote health equities, reduce healthcare inequalities and improve the health and well-being of the population. That’s the reason why we are doing further edu
