NUR 513 Describe the steps of the evidence-based research process and the importance of using them

Evidence-based practice (EBP) is emphasized at all levels of clinical decision-making. Therefore, it is fundamental for nurses to be well-versed in what is involved in the EBP and how it is implemented. Before formulating the practice change proposal it is essential to determine the appropriateness of the practice problem using databases such as Joanna Briggs Institute (JBI) EBP and Lippincott Procedures (Grand Canyon University Library, n.d).   Nevertheless, there are five key steps involved in the evidence-based research process (American Nurses Association, 2023). The first step is to ask a question concerning an identified nursing issue. The question is formulated in PICO (Patient, Intervention, Comparison, and relevant Outcome) format to have focused research (DeNisco, 2024). The second step is to find the answer to the PICO questions from relevant databases (examples of Databases relevant to nursing include CINAHL and PubMed). This is done by using keywords derived from the PICO question. This helps locate peer-reviewed journal articles relevant to the research topic. For the latest evidence, it is essential to filter the search to be within the last five years.

The third step involves evaluating the validity and applicability of the accessed evidence. This includes checking the design (RCTs are the best), the methods (selection of the participants, randomization, intervention, control, etc.), and the results’ consistency (DeNisco, 2024). After appraising the evidence, the next step is to integrate the evidence with clinical expertise and patient-specific context to make clinical decisions (DeNisco, 2024). The outcomes are then assessed to confirm the effectiveness of the intervention and if there is a need to further improvement.

Evidence-Based Practice Proposal: Health Informatics – Medication Error

Evidence-based practice (EBP) is the careful and well-advised application of contemporary best evidence in concurrence with clinical knowledge and patient ethics. One of the examples of EBP is the piloting of health care determinations through the use of health informatics in mitigating medication errors. The evidence-based practice procedures have five cardinal stairs: Constructing the clinical question, where the nurse constructs applicable and an accountable clinical question based on the patient’s manifestation (Escoffery et al., 2019).  Similarly, at the second step of probing for the best available and accessible evidence, the nurse chooses helpful resources and performs a search. At stage three, an astute analysis of evidence to ascertain rationality and practicality are done to determine the validity and the practicality of the research. In addition, the fourth stage involves the amalgamation of the appraisal with personal clinical knowledge and the patient’s predisposition to make a decision to mitigate a patient’s case. Further, the last step of assessing the process to improve it (Iowa Model Collaborative et al., 2017).

The Importance of Using the Steps of Evidence-Based Practice

The application of evidence-based practice makes certain clinical practices based on solid evidence and patients have satisfaction. In addition, using evidence-based practice translates into more harmonious clinical suggestions and practice over the health system. Besides, the evidence-based practice also recognizes that patient ethics, choices, and situations are taken into account to increase the probability of good services results and not harm. (Fisher & Bloomfield, 2019). In the initial stages of an EBP project, nurses in the specialty of health informatics locate sources of information from online patient portals and electronic medical records (Fisher & Bloomfield, 2019). Examples of specific sources of information include American Health Information Management Association (AHIMA) and American Nurses Association portal, among others.

The first step in the Evidence-Based Practice (EBP) approach is identifying a clinical problem or concern. Even though it appears simple, there must be a solution to cope with this issue or collection of issues. The PICOT method is useful for providing guidance while coming up with research topics (DeNisco & Barker, 2016). The letters “P,” “I,” “C,” “O,” and “T” stand for target population, intervention or variable, comparison of intervention or variable with and without intervention or variable, outcome, and time required to achieve the outcome.

The following step in EBP is research. After creating a PICOT, look for any relevant historical and current evidence. There are also research steps included. You must have inclusion and exclusion criteria, which are frequently based on the ages of the participants in the study. You must carefully examine the various types of evidence before deciding which databases to search. The findings of observational, descriptive, or qualitative studies differ significantly from those of systematic reviews or meta-analyses of randomized controlled trials. The AACN, according to DeNisco and Barker, has developed a new method for effectively rating evidence (2016, p. 476). New data must be thoroughly reviewed to determine whether a study is credible, of high quality, and relevant to your EBP. There are many frameworks, checklists, and models to choose from.

Re: Topic 7 DQ 1

Identifying a clinical problem or question is the first step in the Evidence Based Practice (EBP) process. Sounds simple enough, but this problem or questions must be answerable. The PICOT method is extremely useful for guiding the development of research questions (DeNisco & Barker, 2016). The “P” stands for the target population, the “I” stands for the intervention or variable that is being studied, the “C” stands for a comparison of with and without said intervention or variable, the “O” stands for the outcome, and the “T” stands for the time it takes to reach an outcome.

The next step in EBP is research. After forming a PICOT, you want to search for all existing and current evidence related to your topic. There are steps related to research as well. You must have inclusion and exclusion criteria; this is often related to age of study. You must select appropriate databases to search, and you must critically analyze the evidence types. Not all evidence is created equally, there is a major

difference between evidence from a systemic review or meta-analysis with randomized trials versus observational descriptive or qualitative studies. According to DeNisco and Barker, the AACN has created a new evidence leveling system that essentially grades the evidence (2016, p. 476). Critically analyzing the newfound evidence is important to determine if the research is valid, of quality and applicable to your EBP. There are a number of models, checklist, and grading systems available to help with the critical analysis and organization of evidence (DeNisco & Barker, 2016). There are a number of databases that can be used to research, the GCU nursing library page lists several nursing databases that are great for the initial stages of EBP and research (GCU Library, n.d.). To name a few: CINAHL Complete is a good data based to start because it has quite a few specialties and is very nursing specific. PubMed is another one that has a gigantic database and is free to use. And COCHRAN library is a good one because it has seven different databases that related to providing medical providers with high-quality and independent evidence that can help with decision making in care.

Nursing informatics will also play a significant role in the future of advanced registered nursing practice. The argument is that the scope is one of the most powerful, as well as one of the most far-reaching and current care practices. Furthermore, NI entails the use of evidence-based practice (EBP) in nursing care to improve patient care delivery (Sewell, 2018). A framework for documenting clinical systems and delivering healthcare models is provided by the nursing informatics software platform. Furthermore, an advanced registered nurse specializing in informatics gains a deeper understanding of the healthcare industry’s design and organizational framework, particularly by focusing on the sector’s current challenges.

A care provider must have a Bachelor of Science in Nursing (BSN) or a master’s degree to specialize in the field of nursing informatics. Nonetheless, a nurse in this specialty must have a Nursing Informatics Certificate in order to implement clinical information systems in a health facility. Furthermore, nurses who specialize in NI must have academic credentials in both CAHIMS and CPHIMS (Certified Professional in Healthcare Information and Management Systems) (Gaber, 2016). A nurse must also have strong computer skills and be familiar with computer software programs such as Microsoft Word, PowerPoint, and Excel. A nurse must also understand how the clinical workflow works in a care facility, from inpatient to outpatient settings (Sewell, 2018). Before pursuing the specialty of nursing informatics, a nurse must have a thorough understanding of the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) standards. In order to obtain a scope of practice, nurses interested in the informatics specialty must also be credentialed by the American Nurse Credentialing Center (Liu, Lee & Mills, 2015). A nurse specializing in informatics, on the other hand, should gain a deeper understanding of the Health Insurance Portability and Accountability Act (HIPAA) requirements in order to gain insight into the safety regulations to safeguard medical information.

Hello,

I agree with you that when developing an EBP research topic, it is critical for the nurse to conduct a PICOT analysis, which will be critical in determining the feasibility of the research problem. The nurse requires adequate information about the research topic in order to understand the larger scope of the area of study and to be able to build on the already existing knowledge on a given research topic. These checks must be performed prior to carrying out the EBP so that the research work produced has a practical application in promoting quality care among the target population (Li et al., 2019). Some of the pertinent questions considered in the PICOT include: Will the study’s findings benefit the target population? Will the discovery be applicable in practice? Will the discovery support an existing concept or practice, or will it lead to the development of a new theory? Will the findings support the current nurse theory or provide evidence for a change in the current nurse theory? These are the key areas that the EBP research problem should address; otherwise, the problem must be re-examined and adjusted in order to have clinical relevance and importance in nurse practice (Duff et al., 2020).

Loosing your soul in nursing is not a difficult task and happens silently, overtime. Then all of a sudden something triggers your self-awareness that something is missing, your soul. In nursing its is no easy task to lose yourself and takes a concerted effort to not give to much of yourself. I have been burnt out more then once in my career. I have spent over 2 decades working prehospital, emergency room, and ICU. In this time period I lost my soul a couple of times. I have reinvigorated my flame for nursing. After very difficult personal experiences and evaluating what is important to me I now longer work in the high intensity environment. I have changed environments and work as a charge nurse in a specialty clinic. I am able to continue to have a positive impact on patient care minus the tons of stress. Not to say there is no stress but a lower intensity and a lot less. Change can be good but is a stressor all int’s own. One of the many positive things about our profession is the ability to change environments. This flexibility allows us to preserve or recapture our souls when they try and fly away.

Re: Topic 7 DQ 1

Evidence-based practice (EBP) unites research evidence with clinical expertise and encourages individualized care by including patient preferences (Stevens, 2013). A major paradigm shift in the way nurses began to think about research results, framed the context for improvement, and employed change to transform healthcare was apparent with EBP on the scene. Patient care outcomes substantially improve with the use of EBP and tested interventions.

Two nursing educators and researchers, Melnyk and Fineout-Overholt, suggest seven steps in the EBP process, all proving vital for effective implementation and integration into the clinical setting (Melnyk & Fineout-Overholt, 2011, as cited in DeNisco & Barker, 2016, p.465). In the past, it has taken years to transfer new knowledge into practice, so speed is the primary goal for EBP models (DeNisco & Barker, 2016). Many models exist and common aspects include: identifying a problem and having a spirit of inquiry, asking practice questions, reviewing the latest evidence, critically appraising the evidence, integrating personal expertise and patient preferences in and grading, and finally organizing pieces in a meaningful way.

In the initial stages of locating information, I would start searching in PubMed because it is a free database accessible from any mobile device (DeNisco & Barker, 2016). Begin with a researchable and answerable question is essential. The PICOT question formulation method is widely used for clinical problems. Searching databases such as CINAHL, The Cochrane Library, or MEDLINE would be my next approach. The CINAHL database provides access to over 3,000 nursing and allied health journals yet, a subscription is required for access.

Nurses within leadership have an unprecedented opportunity as mentioned by Stevens (2013), to step forward and transform healthcare from a systems perspective, focusing on EBP for patient safety, patient engagement, and clinical effectiveness. The “next big ideas” in EBP focus on making systems improvements and transforming healthcare. Moving beyond the individual and shifting the focus on groups, health systems, and the community is one part. Dissemination and implementation of strategies to assure effective and safe patient care are delivered rapidly is the second part. While there are benefits to the evidence gathered and applied, the true benefit goes to the patient.

I agree with you that the nurse leaders and nurse managers have a responsibility of driving change in the nursing practice by promoting the successful implementation of the EBP project findings into the nurse practice. However, for the EBP project to have significant relevance to the nurse practice it is imperative that all the steps required in conducting an EBP are properly adhered to. The problem of research should be a clinical or a nursing area of concern whose solution can be arrived at through carrying out a viable research project that will generate new knowledge and nurse practice or build on the already existing bulk of knowledge (Li et al., 2019). The research objective should be SMART, that is, it should be specific, and target specific area of nursing or population, measurable, achievable, relevant to the area of practice and time bound. I do also agree with you that there are a number of sites that one can use when doing research on the topic or nursing area of research to focus on. The nursing data bases such as CINAHL, EMBASE, ERIC, MEDLINE via PUBMED or via Ovid, Cochrane Library, HAPI and Nursing Reference Centre Plus. These databases contain sufficient nursing research work that one can build on when developing and EBP research question to ensure its relevance and usefulness in the nursing practice (Hopia & Heikkilä, 2019).
