Accordingly, nurses need to be prepared educationally to manage clinical risk effectively when delivering patient care (Johnstone, 2007).   Working as an ostomy nurse, I have witnessed nurses working in their units catching medication errors early and/or preventing a patient’s pressure injury from worsening or developing by following preventive measures and placing a wound care consult and I have also witnessed the opposite. With these examples in mind, if they had quality and risk management education in their curriculum, new nurses would be knowledgeable starting their careers. They will be more prepared educationally to discover and prevent risks/adverse events in health care and thereby contribute to the broader agenda (locally and globally) for safety and quality in health care (Johnstone, 2007).

This course examines nursing theory and the role of ethics for advanced registered nurses within the Christian worldview and through a leadership perspective focused on improving health care outcomes. Over time, the role of the advanced registered nurse has continued to transform. Per the Consensus Model for APRN Regulation, roles currently recognized as advanced practice registered nurse (APRN) include nurse practitioner, certified nurse-midwife, certified registered nurse anesthetist, and clinical nurse specialist. The earliest serving role dates back more than 150 years to the American Civil War where the Nurse Anesthetist treated wounded soldiers with chloroform. This role continued throughout many wars, then evolved into rural areas. As of 2021, there were 43,950 certified registered nurse anesthetists practicing throughout the United States. Next, the first mid-wifery school began in 1925, focused on improving maternal and infant outcomes. As of 2019, there were 11,826 certified nurse-midwifes providing care for women during pregnancy, childbirth, the postpartum period, care of the newborn, family planning, and gynecologic needs. In the 1960’s, the nurse practitioner role was developed to serve pediatric and underserved populations due to the lack of primary care physicians. As of 2022, there were 355,000 licensed nurse practitioners in the United States serving in specialty roles such as adult-gerontology, family, acute care, pediatric, women’s health, and psychiatric care. Also during the 1960’s, the clinical nurse specialist (CNS) role began in effort to serve the growing need for care of the psychiatric population. While there were once many clinical nurse specialists, cost containment forced many hospitals to move away from supporting this role due to lack of evidence that they decreased hospital costs. They were also faced with the lack of ability to bill for services provided (DeNisco, 2023).

The American Association of Colleges of Nursing (AACN) defines APRN as “any form of nursing intervention that influences health care outcomes for individuals or populations, including direct care of individual patients, management of care for individuals and populations, administration of nursing and health care organizations, and the development and implementation of health policy” (DeNisco, 2023, pg.6). Therefore, AACN also recognizes nurse administrators, public health nurses, nurse policy makers, nurse educators, and clinical nurse leaders as advance practice nurses. While some of these roles only require a baccalaureate or master’s degree, there is a push to pursue a doctorate in nursing practice to be considered an APRN (DeNisco, 2023).

APRN roles will continue to grow in the future to provide care to the community due to the increasing focus on cost containment, lack of primary care physicians, and the need for high-quality care.  For example, the Bureau of Labor Statistics projected  in 2022 that by 2029 “the NP role will have grown by 52% compared to 10% for physicians. In 2011, the Institute of Medicine (IOM) released a report that identified the need for nurses to be placed at the forefront of health care” (DeNisco, 2023, pg. 22). I expect new roles will continue to evolve out of the need for guided care, more complex disease processes, the aging population, and the need for health promotion and disease prevention.

The role of advanced registered nurse has undergone significant transformation over time. The transformation can be seen in the changes in roles between the 20th and 21st century. The transformation in roles is largely attributed to the influence of factors that include professional, government, legal, and technological changes. The advanced registered nurse did not rely mainly on health technologies in the 20th century. This changed in the 21st century where novel healthcare technologies are largely utilized to provide high quality, safe and efficient care to diverse populations. The 20th century nurse also provided fee-for-service care. The focus of nursing care was largely on the quantity of care given to the patients. Patients also incurred significant expenses in seeking the care that they needed. The focus of nursing care in the 21st century is however different from the above. Advanced registered nurses in the 21st century provide value-based care. The focus of nursing care is on the realization of quality and patient-centered outcomes (Gullatte, 2018).
