You have been asked to investigate a new procedure that physicians would like nurses to adopt in the hospital. Discuss the process you would apply to determine whether the procedure falls within the RN scope of practice and how you would go about introducing the new procedure with physicians and fellow nurses.

The practice of nursing is in a constant state of motion and in an effort to create guidelines for standards of practice, a “list of competencies to help nurses better understand the expectations…these include assessment, diagnosis, planning, coordination of care, health teaching consultation, prescriptive authority, and evaluation” (Dean, 2018). The National Council of State Boards of Nursing (NCSBN) assists all fifty states, including Oklahoma, in developing guidelines for standards of care for its Board of Nursing (BON) (ncsbn.org). These guidelines set forth by the NCSBN encourage the state of Oklahoma to use evidence-based practice in its Nurse Practice Act (NPA) which promotes patient safety and outcomes (Huynh, 2021).

With all states using the NCABN, a sense of continuity and standardization can be felt in the profession of nursing. With developments in medicine and other interventions, it is vital in the nurse role to understand what his/her role is in regards to patient safety and outcomes. By using the standards of practice put forth by the state of Oklahoma’s BON, the safety of patients is in the forefront of the care that is provided.

In the post-anesthesia care unit (PACU), the five components of the nursing process are utilized (Dean, 2018). An example of this would be a patient who presents to the PACU after a shoulder arthroscopy. An initial assessment of the patient shows the patient to be moaning, wincing, as well as tachycardia and hypertensive; this is the first step in the nursing process. The second step would be to provide a diagnosis. What is the problem? The patient reports pain (as 8 on a scale of 0-10) as a result of surgery as seen by increased heart rate and blood pressure as well as pain score. The third step is planning/outcomes, what can the nurse implement as far as interventions and possible pharmacologic strategies to alleviate the patient’s pain and what could the outcomes of said implementations be? The fourth step is implementation. The nurse carries out the intervention that was previously evaluated, in this case the nurse decides to dim the lights, quiet the room, and provide the patient with pain medication per orders given by provider. The last step is evaluation. Did the intervention work? Is the patient’s pain alleviated? This step often requires another assessment to determine the effectiveness of the intervention. The Standards of Practice set forth by the BON guides this nurse process in that the nurse is able to evaluate the problem, consider options for implementing interventions then evaluating the outcomes, all based on the appropriate standards of care put in place.

As the country was growing and the population and cities began to increase, nursing became a growing and demanded profession. The United States Constitution does not include provisions related to the booming nursing practice; thus, the responsibility to define and regulate the nursing practice is decided by each individual state.  In 1903, North Carolina was the first state that a nurse registration law was created. The goal was to validate the nursing profession in the public eye while improving nursing education standards (Russell, 2012).  From there, each state was able to create their own outline of nursing scope of practice and standards. The process for the development of the nursing standards in California starts with the Nursing Practice Act (NPA). The Board of Registered nursing is required by the NPA to outline and dictate the standard scope of practice for nurses in California. The NPA is published by LexisNexis and is updated every year (California Board or Registered Nursing, 2022). In 1905, the power to administer exams, approve educational programs, issue certificates, and revoke certificates of registered nurses was given to the University of California Board of Regents. By 1927 the Buraru of Registration of Nurses was created and placed within the department of health. By 1975 The board was known known as the Board of Registered Nursing and the Nursing Practice Act was amended to provide the discretion of nursing (California Board of Registered Nursing, 2022).  California laws and regulations are created to keep both the nurse and patient safe. Following these guidelines are essential in the everyday nursing process, for they guide us in our scope of practice to provide quality and safe care. Nurses in California are independent health care providers, so if we are working within our scope of practice, defined by the state, we are not required to be supervised. Standards of practice influence my nursing process daily guiding me to where I can and can not act. For instance, I am able to apply nursing diagnosis then create and implement a plan of care for said nursing diagnosis; however, I am not able to provide a medical diagnosis for that is not within my scope of practice.

Ethical principles are crucial in guiding ethical practice in particular situations. Nurses need to specify and explain their professional ethical obligations to patients and assure them to be ethically competent and trustworthy. The ethical principle I have selected is accountability. Accountability characterizes the idea that a person is responsible for their actions (Chesterton et al., 2021). If a person opts for unfavorable actions, the person must be prepared to face the consequences. Accountability is majorly about protecting quality patient care outcomes, maintaining competency, and safeguarding standards of practice while taking responsibility for those affected by nurses’ clinical practice. Every nurse must be responsible for his or her actions and choices in clinical practice (Rubio-Navarro et al., 2019). Accountability seeks a high work level, encourages dependability, fosters honesty, and gains trust from clients. Accountable nurses often provide quality patient care. Accountability can be demonstrated in patient situations by administering drugs as prescribed and documenting all supplies utilized in patient care.
