The goal of health promotion programs is to get people and communities involved in and empowered to choose healthy behaviors and make changes that make it less likely that they will get chronic diseases and other diseases. The World Health Organization defines health promotion as: enables individuals to gain more control over their health. It encompasses a wide range of social and environmental interventions that aim to address and prevent the underlying causes of illness rather than just treat and cure, thereby enhancing and safeguarding individuals’ health and quality of life.

I did not know about the term sacred cows until now. I found the topic interesting, and it instantly reminded me of how we were taught in nursing school to routinely check gastric residual volume. However, I was taught differently at my work. They sent out a change of practice/policy announcement stating that residual volume check must only be done if ordered by the doctor and not routinely performed, tube feedings must not be held if gastric residual volume check is ordered, and must be discontinued if tube feeding is tolerated at goal rate. The rationale behind the changes are because the measurements are highly subjective and have never been validated or standardized, gastric residual checks may lead to clogged tubes and underfeeding, there is no correlation between residual checks and aspiration pneumonia or infections. I have seen some nurses who still do it just because they have been used to doing it. I think that’s the challenge with some experienced nurses (including instructors)–they are so used to doing things the same way that they do not question the practice anymore or make an effort to break habits.

As a future nurse leader, I would make sure that everyone complies with the policies of the unit or facility. These policies are put in place because they were backed up by evidence, and that they are aimed at patient safety and favorable outcomes. Another thing that I would address is the old-school culture of experienced nurses “eating their young”. Some tenured nurses are mean to new grads/new hires or boss them around, and people do not do anything about it because it has been a norm. For me, it should not be a normal thing. The workplace must be healthy environment where people treat each other with respect and support each other. The working environment must be a safe place for all. Being a nurse is stressful enough, no one needs added stress caused by toxic coworkers.
