Five basic categories comprise SDOHs: health and health care, education, social and community context, economic stability, and neighborhood and physical environment (Clements, 2018). In the last assignment, an evaluation of Family Y, an African-American lower-middle-class family, was done. The goal of this research is to examine the SDOH related with the family’s health status, to offer age-appropriate screening procedures for family members, and to investigate a health model that may be incorporated into an action plan.

Effects of SDOH on the Family’s Health

Family Y’s health is affected by education, economic stability, health and health care, and the neighborhood. Education, according to Zajacova and Lawrence (2018), contributes to better, more stable sources of income that generate a higher income and enable families to amass wealth used to improve their health. In addition, the family resides in a safe neighborhood with access to sufficient clean water, nutritious foods, healthcare facilities, and transportation. This has had a significant impact on the family’s health, as they are no longer susceptible to water-borne and water-washed diseases. Additionally, access to healthy foods reduces consumption of unhealthy foods, which reduces the risk of lifestyle diseases (Clements, 2018). Access to healthcare facilities has allowed the family to obtain essential and specialized healthcare services, thereby enhancing their health.

Family Y members are susceptible to chronic diseases due to risk factors such as a history of diabetes and hypertension in the family and their race. Therefore, age-appropriate screenings are essential for detecting chronic diseases at an early stage and implementing the appropriate interventions to treat or slow their progression. Mr. Y (62 years old, type 2 diabetes and hypertension under control) should undergo colorectal cancer screening.

The United States Preventive Services Task Force (USPSTF) suggests colorectal cancer screening for adults aged 45 to 75. (USPSTF, n.d.). Since he is at risk for heart disease, cholesterol testing is also required. Due to the risk of cataracts and diabetic foot ulcer, an annual eye and foot examination is also advised.

Mrs. Y (59 years) should undergo screenings for blood pressure, blood sugar, colorectal cancer, cholesterol, cervical cancer, and mammography. The USPSTF recommends screening for hypertension beginning at age 18 and diabetes beginning at age 35. In addition, it recommends cervical cancer screening for women aged 30-65, including three-yearly cervical cytology testing and five-yearly high-risk HPV testing (USPSTF, n.d.). In addition, the USPSTF recommends biennial screening mammography for women aged 50 to 74. The recommended screenings for the daughters (ages 35, 32, and 28) are blood pressure, blood sugar, monthly self-breast examination, biannual dental examinations, and cervical cancer screening. According to USPSTF recommendations, the appropriate cervical cancer screening for a 28-year-old is three-yearly cervical cytology alone (USPSTF, n.d.). Health System

The Health Belief Model (HBM) was chosen to assist in the development of an action plan. HBM forecasts and explains changes in the health and lifestyle behaviors of individuals. In addition, HBM has described the key factors that influence health behavior, such as perceived susceptibility, severity, benefits, barriers to action, cues to action, and self-efficacy (Bechard et al., 2021). These elements emphasize an individual’s beliefs regarding health and disease, thereby influencing health-related behaviors. HBM is the optimal health model for Family Y because it can predict the health behaviors of its members. In addition, the model can reveal how members’ beliefs about disease and health influence their lifestyle choices (Bechard et al., 2021). Considering that Mr. Y had diabetes and hypertension, Mrs. Y and her children recognize that they are susceptible to developing lifestyle-related illnesses. As a result, they have adopted healthy lifestyle practices, such as regular physical activity and a healthy diet, which they perceive to reduce the risk of disease.

Family-centered health promotion measures

The HBM can be applied to family health promotion by first identifying the family’s health needs through a needs assessment. Additionally, the evaluation is essential for determining the prevalent health risks among family members. The next step in health promotion using the HBM is to assess whether family members believe they are at risk for any diseases based on the health risks and how seriously they perceive them to affect their health (Bechard et al., 2021). The third step is to inquire about the family’s interventions to mitigate health risks, their perspective on how they will benefit their health, and the barriers preventing them from effectively implementing the interventions. At this stage, additional interventions that can help mitigate health risks should also be provided.

To achieve the desired outcome, the family should be assisted in identifying strategies to eliminate these barriers. Included in the health promotion communication strategies is involving the entire family by allowing each member to participate in the discussion (Barnes et al., 2020). In addition, the provider should recognize the family’s right to privacy and independence in carrying out their responsibilities.
