In California, one of the disasters we are continually preparing for is an earthquake.  Unlike hurricanes which can have a warning of a few days or a tornado which can strike with a few minutes warning to seek shelter, earthquakes strike without warning and people must seek whatever shelter is immediately available to them (usually within a matter of a few feet like desks, doorways, etc).  Efforts like The Great California Shake Out encourage students to be prepared for an earthquake by using their desk to provide safe shelter in the event of falling debris such as lights, ceiling tiles, and even roofs.

The community setting for discussion here is the public school and the School Nurse is the nursing role.  In conjunction with exercises like the Great California Shake Out, the School Nurse should evaluate the shelter available in each classroom to confirm there are enough desks or tables for each student, confirm the proper way for students to secure themselves, and identify any special hazards in the room.  In addition, the school nurse should evaluate emergency supplies and help establish a plan should the school be cut off from outside resources or parents be cut off and unable to reach their children.

One key resource outside the school for the School Nurse to collaborate with would be the local EMS operator.  The school nurse should understand what resources are available for on-site treatement and transport of injured students, as well as how best to communicate needs and triage patients prior to EMS arrival.  This will speed up the treatment of the most critical patients.  It will also be important to establish how EMS providers will check on the school if traditional communications such as phones are inoperable.  This is very common in earthquakes since even if power and phones are operating, often times the networks will be overloaded by people calling to check on loved ones and jamming all available phone lines.

A disaster is any event that causes level of destruction, death, or injury that affects the abilities of the community to respond to the incident using available resources” (Nies, 2019). My community was hit several years ago with the Superstorm Sandy and hurricane Irene. This caused damaging winds, floods, power outages, and left people homeless. The American Red Cross was available in the community to provide food, shelter, and necessities to families who needed it most. Several people did not have power for weeks on end, causing them to need a place to take care of their hygiene needs and basic essential living needs. “The Red Cross places great emphasis on preparedness and participates with communities in developing and testing their disaster plans, maintaining and training personnel for disaster response, and responding during an actual emergency or disaster” (Nies, 2019). The  ARC opens shelters to ensure people have a safe and reliable place before, during, and after the storm. Different team members and caseworkers from the Red Cross meet with some individuals affected by the disaster to offer assistance. They also offer relief items which include toothbrushes, toothpaste, cleaning supplies, flashlights, batteries, trash bags, cold-weather essentials as well. 

Disasters happen anywhere and at any given time. Everyone is affected by disasters in different ways. It affects multiple settings in a community, one particular setting that is impacted would be the elderly population. I feel as though the elderly are affected in a more vulnerable way and that’s why the community needs to make sure the elderly are prepared for disasters to come. According to the CDC, elders may experience challenges that come with advanced age, such as hearing, or vision, or cognitive impairment, which may make it difficult to access, understand, and respond to emergency instructions. The community needs to come together and educate the elderly population on what to expect when a disaster hits. For the elderly that live in nursing facilities, the nursing staff can provide educational videos and handouts. For the elderly that live in other communities, nurses can stop by to one of the Senior Citizens meetings that take place in my town. They can offer them hygiene kits, first aid kits, and educational supplies. They can make sure that each elderly is receiving food and has shelter as well. 

Preparing for disasters is an interdisciplinary approach that involves more than just one person and one role. A community health nurse could collaborate with EMT’s, police offers, and really any first responder in preparing to educate the elderly on disasters and how to respond properly. Being prepared and educated is the priority when it comes to disasters.

Think about disasters your community has or could potentially face. Now, think about how your community is helping its members prepare.
