Nursing in the early part of the 20th century changed drastically from the previous vocation that it had been during most of the 19th century. One area where evidence in nursing care changed during the 20th century is the way of caring for patients today was the development of Intensive Care Units.  This specialty was born in Copenhagen in 1953 where the award was solely dedicated for each patient to have their own nurse in order to treat the polio epidemic in which more than 300 patients per week were being admitted and 10% of them required respiratory support.  By dedicating the award to care for these patients, mortality from polio decreased from 80% to approximately 40% (Kelly, Fong, Hirsch, & Nolan, 2014).

Great post! It was very informative and gave me the information I hadn’t realized, not so much that I didn’t know, but I have always thought that ICU and Cardiac Care Units came hand and hand. It always amazes me at how truly recent these types of advances in medicine, and the care of the sick, is. Nursing has been around for some time, but we are very much early in our developments in nursing and medicine, and our care of our patients. One example that comes to mind, is how many people we lost early with Covid and on a vent, but today we are seeing a higher survival rate. Somewhere, someone said let’s try laying the patients on their stomachs – and it worked! Having the specialty units is so important for these types of trials and errors, as the nurses can better care for 2 patients who are deathly ill, and they can monitor their status much easier, allowing them to try new ways, and monitor if those different interventions can be successful.

I also did mine on Certification, and how important obtaining one can be. No matter what job we are doing, we need specific education for that role. Whether that role is a chef, a cashier, an environmental service worker, a manager, the CEO of a company – every role has a set of guidelines and rules to be followed in that role. So why should this be different in nursing? One of my favorite things about healthcare is never-ending education and the long list of opportunities. When Nightingale studied to be a nurse, that was it – you stayed by your patients, ensured they, and their living area, was clean and did what the doctor told you to do. Today has that changed! As we all know, medicine is a very deep and complex topic. Every nurse has one aspect of the body, that fascinates them, or an area within the field that does. There is a large difference between an ER nurse and a med/surg nurse, and OR nurse from a flight nurse. We all started with the same education. The pre-requisites, the nursing classes, taking the dreaded NCLEX. However, just like the training for a chef is different than a CEO, so is every aspect of nursing. Certification allows the nurse to be able to obtain more in-depth, and specific, training for that role. The nursing school covers the basics. However, you can’t walk out of the testing center knowing you passed, and jump on a helicopter to be a flight nurse. There is a need for specific training and understanding of policies and procedures when taking care of patients that are critically ill. Certification has many benefits, but in those benefits, there is always a downside. With so much education in a specific area, we tend to forget that there are other aspects that need care as well. I may be great at lung sounds, knowing from the sound what that patient needs now, and what they need long term, for the best chance of lung recovery. However, I don’t know if those interventions will affect another aspect of their care. For example, if I specialize in respiratory and hear nasty lung sounds, I’m going to start antibiotics, maybe some steroids, a diuretic. However, I don’t know what their skin is like – and implementing certain medications can actually make that patient worse in terms of their skin. As with anywhere in medicine, this is another example of the true importance of collaboration with the entire team, especially in a critically ill patient. Of course, there is also the time and money spent on obtaining the certification, and the maintenance of keeping it active.
