I’ve worked with a wide range of patients from birth to geriatrics. Studies have shown that the young babies and the elderly are more likely to become victims of abuse. While I was a PCA, I was assigned to a sitter case with an elderly woman with dementia. She was sweet as pie, but very forgetful and a huge fan of James Brown. Her sister and daughter were her primary caregivers until she was approved for a nursing facility. Some of the findings that did not sit right with me, were the “jokes” they were making. Some of the “jokes” were about her dying soon, a smothering with a pillow “joke”, etc. Outside of physical abuse, the elderly/disabled are easily subjected to financial abuse, especially when they are fully dependent on their caregivers. Some findings that would alert me to violence are bruises that are healing at different stages and that are in random spots on the body. Another finding is the patient flinching or seeming timid when being approached.

Screenings are so important when it comes to violence. Upon admission, we have to ask these question for physical, financial, sexual abuse. At one point, I would just ask the questions, but I’ve learned to look at them when I ask. Sometimes one may say “no’, but their body language and physical assessment shows the opposite. I believe for the most part they are completed in detail, if the answer is a yes, then there is a prompt for more information.
