The geopolitical aspect of a community is defined by physical traits like where the community is located, who and what lives in the area, and what has been built by the population (Green, 2018). The phenomenological aspect of a community is described more in the nonphysical aspects, such as, how the population lives (Green, 2018). Both ideals are important to a community assessment. They are the objective and subjective aspects of the analyzing component of the assessment. Without these data points issues cannot be identified. Interventions cannot be incorporated without an issue to solve. It would be like looking at a seemingly healthy person in a doctor’s office having no data as to why they are there. A nurse would not have a starting point. They could make up a diagnosis and a plan, but it would more than likely not be what the patient came in for, therefore it would not serve the patient.

The nursing process starts with gathering data. The community health nurse must assess the community by gathering information personally, through stakeholders, or through research.  Once an issue is identified or diagnosed, such as air pollution, the nurse can develop a plan to correct the problem, or at least assist the population in ways to breathe cleaner air. The steps the nurse comes up with to remedy the problem is the intervention phase. In this case, it could be announcing the best time of day to do outdoor activities. Or, providing masks for individuals with lung disease. After these ideas are put into action, the nurse can evaluate the outcome for the community. Such as, did fewer people suffer from asthma attacks during this time frame. If the goal had not been attained the nurse would continue the cycle until the most amount of people possible, were breathing clean air and there was a reduction in lung disease.

The nurse could use community resources for announcing the interventions. The local news station could run a story on the subject. A non-profit organization could donate masks to those in need. A spiritual organization could help spread the news about the best times to be outdoors. An individual with a Christian worldview would spread knowledge with love and kindness to their neighbors.  They would help those in need to protect the lives of others, as they are a valued gift from God.

Community health nurses help develop strategies to help with health promotion for an entire population of groups, families, or individuals (Kiper & Geist, 2020). They focus on care outside of the hospital level and provide their own assessments that incorporate health needs and resources that are available to the individual or groups of people. Because community health nurses provide care in areas such as clinics, community nursing centers, local and state health departments, homeless shelters, works sites, etc, it is important for these nurses to build relationships with what are called stakeholders.
