Sustainability Plan Worksheet

An improvement team needs to consider long-term sustainability of an improvement effort. This worksheet will help you outline the elements needed for a sustainability plan for your proposed evidenced-based quality and/or safety program design by taking you through a series of questions. By answering these questions, you will be able to develop a plan to ensure the longevity of the expected improvements from your proposed quality and/or safety program stick long-term..

MeasurementAfter project completion: ·       What will you continue to measure? ·       What will you stop measuring? ·       What will you do if you see a negative effect? ·       What will you do if you see clinical significance, but not statistical significance?	·       I will continue to measure the glycosylated hemoglobin (A1C) in patients living with diabetes. This measurement would continue to determine the efficacy of the intervention·       I will stop measuring levels of insulin before and after meals. This process would provide enough time to observe the effect of insulin on controlling blood sugar. ·       I will stop the insulin pump intervention on the patients and review their medical history as a negative effect. ·       In situations of clinical significance, I will continue with the intervention as it already has a positive patient outcome (Lamé & Dixon-Woods, 2020).
Ownership·       Who will own the new process that was implemented? ·       How will you tell if the person is engaged and onboard with the improvement process?	·       My healthcare organization would own the new process because they provide most of the resources required for the success of this program.·       The unity of the healthcare team towards the process would show their level of commitment to the success of the process (Kilbourne et al., 2018). Besides, despite their busy schedule, they still take time to help in examining patients undergoing the program.
Communication·       How will you communicate about the change, and who will be the messenger? ·       When will communication take place? ·       How will you support individuals in the new process? ·       What type of training will you use after project completion? ·       Who will be responsible for that training?	·       I will communicate the change using internal memos via employee emails. The approved communication platforms within the facility would as well be utilized.·       The communication will occur after approving that insulin pump intervention can save many diabetic patients. ·       I will support individuals by availing the right information on the proposed intervention and its effect on solving healthcare problems (Lamé & Dixon-Woods, 2020). ·       I will train patients and nurses to effectively use the device and red alerts that should prompt a patient to see a physician. ·       Nurses who have been helping by implementing the problem would help in training nurses.
Change Management·       How would you use your evidence-based change model to ensure adoption and sustainability? ·       How will you standardize the process so that it is easy to do the “new” right thing?	·       It will provide clear guidelines and timelines for implementation and regular updates on progress to keep everyone informed and motivated.·       I will focus on understanding the responsibilities of the team, the quality of the education provided to the frontline staff, and the support of the management team (Barnes‐Daly et al., 2018). ·       I will make all the nurse professionals take the practice as part of their ethical healthcare practice.
Workload·       Is the change increasing the overall workload to the system? ·       If so, how can you decrease the workload? ·       If not, how will you communicate about what is changing and not changing?	·       The change is decreasing the nurse workload.·       The change would reduce the workload on nurses because the patients would be undertaking the interventions without the help of the nurses (Lamé & Dixon-Woods, 2020). This is different from multiple injections where the nurses had to attend to the diabetic patients.
