Well-designed high-level research optimally supports the direct practice improvement project. This type of research adds more value as the evidence presented is mostly reliable due to the study methodology and limited bias during the study. Powers et al., (2021) research is one example of high-level evidence that supports my proposed intervention.  This is a randomized controlled trial that aims to determine the efficacy of computer or video virtual reality on pain. This study uses a large sample size of 103 participants; larger sample sizes increase the change of the study showing statistical significance. The overall results of this study show a 50% reduction in pain after using virtual reality as an intervention. Another article that supports the intervention is Darnall et al., (2020) research on self-administered virtual reality for chronic pain.  This quantitative research is a randomized controlled trial that analyzes data of a virtual reality program that showed statistical significance in reduction of various pain variables.  Both p-values for each study were < 0.001. This further represents high level evidence in that the results from study’s are valid and replicable.

I agree with you that well-designed high-level research optimally supports the direct practice improvement project. Practice improvement project aims at boasting performance levels. Therefore, the project relies on the factual information generated from well-designed high-level research. Research is one example of high-level evidence that supports my proposed intervention (Carretero-Ayuso & García-Sanz-Calcedo, 2018).  Research adheres to scientific protocols of fact-finding. As a result, most studies and research are prioritized in providing credible information. The information allows stakeholders to streamline practice improvement projects.  Practice improvement initiative involves gathering information on the project before deciding (Boaz et al., 2018). Different approaches of research generate detailed information. Therefore, researchers based on the nature of the information they require may conduct different research. Some practice improvement projects may fail to achieve their purpose due to the use of inaccurate information. Percentage errors and p-value are common elements in research. Quality improvement project aims at intervening particular problem. Therefore, research becomes important in direct practice improvement project.

To answer clinical question in order to improve practice, it is imperative that nurses are equipped with the tools to critically appraise evidence (Buccheri & Sharifi, 2017). The two articles named are empirical articles that report the outcome of a study carried out author (s) and based on experiment or observation carried out by the author (s). Further, they are categorized as primary research articles because data are original, and conclusions are provided by the author(s). Bowen and Forrest (2017) noted that empirical articles, also known as primary research, contains the original data and conclusions of the researcher. Additionally, these articles fits the evidence required for level primary research – randomized-Controlled Trial. Here, participants are randomly assigned intervention or control group. Those in the intervention group gets the intervention, while the control or comparison group receives none. Clinician conducting study is unaware which group participants are assigned to which typically leads to unbiased results. The two primary articles are randomized control trials and as such suggest that the intervention of my proposed DPI project is a value add to clinicians and policy makers since it is a lifestyle intervention with evidence of benefit screened detected type 2 diabetes.

JENEVIEVE I concur with you that answering clinical question requires nurses to be equipped with the tools to critically appraise evidence. Nurses are trained and competent workers suitable for addressing clinical concerns. Addressing clinic problems demand the use of factual information that nurses obtain from research. Nursing research allows researchers to obtain accurate information. Healthcare professionals can use different types of research to obtain information that can be used to address different clinical problems (Benedict et al., 2019). Primary research provide original information that can be used in secondary and tertiary studies. Nurses attend to people with different healthcare needs.  As a result, nurses handle various clinical concerns when attending to their patients. Randomized-Controlled Trial allows researchers to randomly assign intervention or control group in their study (Britton et al., 2020). Clinician conducting study is unaware which group participants are assigned to which typically leads to unbiased results. Healthcare professionals with knowledge in statistics are competent in answering clinical question that may arise in their nursing practice.

The two research articles selected to support my Direct Improvement Project (DPI) is a Random Control Trial study article by Gnanakumaram et al (2017). The aim of the article was to evaluate effect of early mobilization program for patients after total knee arthroplasty and impact on length of stay. The instrument utilized was timed up and go (TUG) and data analysis with SPSS package. The second article is a prospective Cohort study conducted by Huang et al (2021). the article evaluated the effects of early ambulation on elderly patients and length of postoperative hospital stay.

These two articles fall into the category of highest level of research with minimal bias and generalization is higher. The articles have specific domains, including study limitations, reporting bias, were precise, consistent and direct in communicating research findings.

Irene I agree with you that minimal bias guarantees the highest level of research. Bias and manipulation of information decreases the accuracy of the research findings. As a result, most researchers use various strategies to minimize exaggeration and manipulation of the research and the outcome (Harrington et al., 2018). Healthcare organizations attend to people with different healthcare needs. Direct improvement project (DPI) benefits from the information obtained from research.

Therefore, accurate research is important in planning and implementing DPI. Random control trial study enables clinicians to use random participants. Communicating research findings to the stakeholders triggers collective participation in the research and implementation of DPI (Eby et al., 2018). Reporting bias to the right players reduces chances of obtaining inaccurate research findings.  Bias may be introduced at different stages of research. Thus, researchers should examine the entire research to identify any form of exaggeration. DPI may suffer the outcome of bias if the manipulation is not detected in time.
