Nurses use a functional health pattern as part of the nursing process. Using a series of questions, nurses can complete an in-depth assessment to help better understand a patient’s overall health and lifestyle. Information regarding functional health patterns offers the nurse significant insight into patients’ current and past state of health (Whitney, 2018). Interviewing patients about their elimination patterns may help with the early identification of clinical manifestations of disease based on changes they experience (Whitney, 2018). The assessment can help to determine if the changes are related to aging or resulting from a more serious medical condition. Nurses play a role in assessing and identifying risk factors for diseases and managing elimination complexities at initial presentation, during treatment, and upon discharge (Whitney, 2018).

The central role of a functional pattern for a nurse includes checkup, diagnosis, planning of a healthcare approach, and execution and assessment of the patient’s data. The procedure helps a nurse apply the best care framework to eliminate waste from the patient’s body. The functional health patterns are vital for the admission and creation of databases for care plans and diagnosis (Khatiban et al., 2019). The nurse uses the information to evaluate the patient’s health before and after treatment plans. Similarly, the data determines areas of improvement to boost the quality of life.

Assisting patients with elimination is an essential aspect of the nurse’s role and has important medical significance as well as psychosocial effects on the client’s quality of life (Dal Molin et al., 2018). As the pattern of healthy bowel movements and urination vary in different patient groups, the management for each patient population may differ. Nurses need to assist with healthy elimination patterns to ensure patients are having regular soft bowel movements and adequate urination and to identify abnormal patterns such as flatulence, constipation, diarrhea, incontinence, fecal impaction, hemorrhoids as well as polyuria, anuria, and other abnormalities which can be signs of underlying medical conditions (Dal Molin et al., 2018). The nursing team must provide strong supportive communication when assisting clients with elimination. A study found that the nurse’s attitudes toward excretion-related nursing care strongly influenced the use of a toilet and the physical functions of the elderly (Dal Molin et al., 2018). Patients may be reluctant to discuss their bowel and bladder problems due to embarrassment. It is vital that nurses maintain open communication and empathy with their clients and ask questions as well as physically assess patients for signs of bladder and bowel irregularities.

While there are pharmacologic alternatives to assist with elimination issues, assistance by nurses is often required. For instance, in abdominal pain syndrome and constipation, studies show that abdominal massage appears to increase bowel function, but without the negative effects of laxatives. Conversely, certain medications can cause constipation, diarrhea, and hinder or exacerbate elimination. Opioids, NSAIDs, antibiotics, anticoagulants, can all induce constipation. It is vital nurses know which patients are at risk for bowel and bladder disruption and monitor them for these issues. The nursing team must provide strong supportive communication when assisting clients with elimination. Functional health patterns can help nurses gain insight and understanding about the current and past state of health for a patient. Incorporating functional assessment improved the nursing process. Utilizing a systematic approach to patient care in examining the patient is the most important part of the nursing process (Khatiban, et al., 2019).

Nursing process includes implementation and evaluation. Early detection of clinical manifestations of disease, utilizing resources for treatment, and foreseeing complications play a major role in restoration of body function. Assessment for history or etiological factors that might contribute to disease is of crucial. Access to early treatment and interventions of illness to prevent worsening of symptoms or progression of disease should be a primary factor in assessment.

Patients should be encouraged to adhere with all medications and recommendations, including follow-up appointments, diet, rest and activity. It is important for the patient and family to learn as much as they can about the disease. Nurses can provide credible sources for reading and printouts for continued education

Functional health patterns include: Assess for/ask about:

  • Health perception -Values, and beliefs, current practices
  • Nutrition Allergies – intake, likes and dislikes
  • Sleep and rest – How many hours of sleep, if broken, insomnia
  • Elimination pattern – Bowel/ bladder, how often, urgency, pain, dysuria, constipation
  • Activity/exercise pattern – Sedentary, walking, exercise
  • Cognitive/perceptual pattern- Mental status, orientation
  • Sensory/perception – Self-concept pattern, feelings
  • Role relationship pattern – What role does the patient take, mother, girlfriend, support, partner
  • Sexuality/reproductive pattern – If sexually active, parnters, history, children etc
  • Coping – Strategies used for coping, (healthy or unhealthy)

Gatheringinformation about functional health patterns can offer the nurse significant insight into the current and past state of health of patients (Whitney, 2018).

A complete functional health assessment can give the nurse great insight into the patients life and how they live from day to day. Learning about coping skills, likes and dislikes… in addtion to those a spiritual assessment, including hope for the future, protective factors and simply asking, “what’s the. most important thing to you right now,” can provide the nurse with useful information. Useful information paves the way for a great assessment and will allow for the Nurse to focus on which there is the greatest need.

Parkinson’s disease is a neurodegenerative, chronic and progressive disease. External signs may be observed, symptoms include muscle stiffness, tremors, slowness and gait disorders. Internally, the body suffers as well. Constipation is the a common gastrointestinal complaint among people afflicted with Parkinson’s disease. It is caused by deterioration of the neurological pathways that promote peristaltic reflex, the reduced peristalsis results in a slower colonic transit time. As Parkinson’s progresses, defecatory dysfunction and decreased bowel movement frequency are present. Many patients also suffer from dysphagia and experience worsening mobility. Poor dietary intake and decrease activity exacerbate the problem. The burden of constipation relates to a negative quality of life, experienced through emotional, psychological, and social areas. The research by McClurg et al., (2016) affirms abdominal massage can reduce the severity of symptoms related to constipation. Abdominal massage stimulates the parasympathetic division of the autonomic nervous system, by administering pressured movements, rectal loading by increasing the motility of the muscles and relaxing the sphincters. The intra-abdominal pressure promotes peristalsis and bowel sensation (Liu, et al., as cited in McClurg et al., (2016). Likewise, the patient with Parkinson’s patient will have urinary symtpoms. As mobility worsens, the ability to leave the house, and nocturia affects sleep. Urinary symptoms are worsened by the movement disorder, as patients are unable to physically respond to the urgency of urinating. Overactive bladder, urgency and nocturia occur Parkinson’s related to dopaminergic mechanisms and its function in maintaining normal bladder control (Sakakibara et al, 2012 as cited in Duncan, 2021).

Nurses can assess of patients and their families with individualized bladder training such as establish a regular toileting schedule, providing education about teaching pelvic floor exercise. Nurses can assist with obtaining necessary supplies and incontinence products (Duncan, 2021). Assessing for family history, early signs and symptoms can identify illness in early stages.
