- Assignments and DQs are not automatically graded (or marked substantive) in the course, so while we try to grade your items as quickly as possible, don’t expect immediate grades to be given at any time.
- While we are off over the weekend, we check our classrooms at least once, each day, over the weekend to answer questions and see what’s up in the forums.
- Please use the Messaging system above( in the purple line ) for communication with your classmates or instructor.
- Late initial DQs are automatically docked 1 point for each day that it is late.
- The participation grade is for me to grade your participation for the week and I do that at the end of each week. The participation grade is not an assignment that you need to do, it is strictly for the instructors.
- If you wait until 11:45 PM to submit your work, don’t be surprised when LopesWrite doesn’t come back quickly. Please don’t panic and try to reach me at midnight. If you are late you just accept the penalty. You should submit before 11:45 PM so this doesn’t happen. Please work as far ahead on assignments as you can and not wait until the last day to get your paper written because that is when many costly mistakes happen.
- I don’t need a long explanation about why things are late. I have 80-100 students at a time and I hear all the explanations. If you need more time, ask. I will probably give you an extension BUT YOU MUST ASK PRIOR TO THE ASSIGNMENT BEING DUE! If your post or assignment is not posted or submitted by the due date I will have to enter a zero when I grade for the entire class. If you want to submit an assignment late you will have to contact me to open the portal up so that you can submit an assignment and then there will be a deduction of 10% per day deducted. For a late post the deduction is one point per day that the post is late. There is no portal for posts you may just post it late and accept the penalty.

Discussion Questions (DQ’s) can be found in the Main Forum.
- The first DQ is due every Saturday at Midnight Arizona Time;
- The second DQ is due every Monday at Midnight Arizona Time.
DQ’s must include at least one in-text citation and accompanying references.
All initial posts which are the Discussion Questions (2) each week have a minimum of 300 words not counting your references.
All substantive posts (3 total) each week must have a minimum of 200 words not counting your references. Substantive posts are those you post when replying to a classmate and do not require a reference but you can use one.
You will have a total of five (5) posts each week. There will also be a Devotional Question each week and a CAT question each week (Classroom assessment technique) If you respond to one of these two questions each week and your reply is a minimum of 200 words it can be a substitute for one of your substantive posts and you will receive credit.
Supporting content can come from the class text or a peer-reviewed journal article, or a credible website. Not one such as Wikipedia and the rule of thumb while constructing your post: if you have to look it up, cite it! ALWAYS!
Finally, as we begin class, I’d like you to remember that all research papers that are turned in must be in proper format and you must use APA 7 the professional/graduate edition for your references. You must have a cover page, a running head at the top left corner of the page that is capitalized and bolded, page numbers, everything must be double-spaced, properly cited and you must have a separate reference page on its own separate page followed by the APA checklist on its own page following the reference page.
I do not teach English. I have posted for you in this forum and in the announcements information on how to set up a research paper and where to locate help. APA 7 professional/graduate edition is not the school APA 7 which is undergraduate APA 7.
You are expected to be in class a minimum of four days per week. If you do not post each time you are online it will not register you as having been online four times during the week. If you want to write posts ahead of time you certainly can on Word and then copy and paste them on separate days in the class but please read all of your classmate’s replies while you are in class, it is required and I don’t like to see all the pink lines of posts that have not been read. Even though you are in class only four times per week you must participate fully in order to learn the material that is covered in this class.

The above verse discusses the importance of nurse leaders who guide us. Discuss the nurse leaders that exist in your professional life and discuss ways your nursing practice has been shaped by their influence. Remember your post must include a minimum of 200 words and include your word count in your post.
In my most recent position as clinical nurse manager for a hospice company I was fortunate to have a nurse leader, my director of patient care services, who in addition to being an RN, is also a chaplain. Her faith drives her in her leadership. Having her as a mentor allowed me in my work environment to express my faith openly and to learn to use it in my role as a leader to guide the nurses who worked under me. She taught me to always consider what my nurses “bring to the table” in any situation. I have been told by my team that while I have expectations of them that they may not always like, they always feel supported, and encouraged. One of the most important lessons that my director taught me, was to recognize each nurse’s individual strengths, and to focus on building those strengths, instead of focusing on weaknesses. We all have areas where we shine, and as a manager, building on those areas, recognizing them in each of my nurses, has enabled me to grow significantly in my role. I have actually found that encouraging them in their strengths and making them feel valued, makes them want to examine where they are weak and they look to me for guidance in making positive changes. , I really enjoyed reading your post. It sounds like you were really fortunate to have such an incredible leader. Some of the characteristics that she taught you that I also truly value and try to see in all of my nurses, is to look at each individual. We are not all the same, nor do we “nurse” the same and this should not be scrutinized. I am glad that nurses “eating their young” is not as prevalent anymore, because this is not a positive or productive learning environment. We all bring different strengths to the unit, and we all learn and precept differently, which is so valuable. These differences should be celebrated and utilized and as you mentioned, then built upon. It is our job as leaders, mentors and experienced nurses to help others grow in their practice. As a nurse educator I really take time to try and get to know my transition into practice nurses before placing them with a preceptor as I know this can make or break a new grad. Hopefully by doing this, it fosters a great learning environment with the type of nurse they need to help guide them and help them grow in their own unique way. Thanks for sharing your experiences
The role of advanced registered nurse has transformed over time primarily by shifting from a role to fill shortage in availability of physicians, to a role with structured education and professional definition. Historically, advanced practice nursing roles including nurse practitioners, nurse midwives and nurse anesthetists came into being as an attempt to improve care access in regions, or specialties, where access to physician care was limited (DeNisco & Barker, 2019). However, these roles, initially had variable educational requirements, and authority often based on region. This lack of role definition was paralleled by other advanced practice nursing roles that did not necessarily have direct clinical contact with patients such as nurse educators, nurse managers and nurse researchers. In 2004 the American Association of Colleges of Nursing (AACN) defined advanced practice nursing as:
“Any form of nursing intervention that influences health care outcomes for individuals or populations, including direct care of individual patients, management of care for individuals and populations, administration of nursing and health care organizations, and the development and implementation of health policy.”
While this definition continues to leave room for variable interpretation, this has served as a point of delineation regarding current and future educational requirements (DeNisco & Barker, 2019). Undoubtedly, the role of advanced registered nurse will continue to be refined by professional and regulatory bodies. The responsibilities of the advanced registered nurse will evolve to continue to meet population health needs, while maintaining strong roots in nursing practice and applications.

I loved reading this discussion question response. I live in a rural area and have seen first hand the impact that Nurse Practitioners serve to both meet clinical needs of our community and to relieve the workload of our local primary care physicians. I’ve recently moved into a new role of nurse educator and I see the benefit of advanced practice nurses across the board. They continue to grow steadily.

These words were spoken by Halfan Mahler, Director General of the World Health Organization (WHO). These words have helped to propel the advancement and development of the nursing profession over time. The nursing practice has change over the years, however, in the 20th century the term “specialist” started to be evolved with the development of more postgraduate courses. With this development came opposition, especially from physicians. However, with the shortage of physicians, advanced practice nurses (APN) were utilized in rural and remote areas as primary healthcare Nurse Practitioners (NP) (Schober et al, 2006).
Over the years however, advanced practice nurse roles and values have been misinterpreted, but with the work of the International Council of Nursing (ICN). APN has been viewed in a different light. Policy makers are seeking creative ways to integrate APN into the healthcare system. However, there are a few challenges that exist. With the healthcare reformation and issues that exist in healthcare stakeholders were forced to proactive seek ways to develop the profession. Through consistency and competency develop APN practices has improved.

The shortage of the primary care physicians continued to drive the demand for the advanced nurse practitioners while also emphasizing on need for them to practice at their full potential (DeNisco, S.M & Barker, A.M, 2024).Initially, this practice was limited to clinical roles such as clinical nurse specialist, nurse practitioner (NP), certified nurse–midwife (CNM), and certified registered nurse anesthetist (CRNA) (DeNisco, S.M & Barker, A.M, 2024). Over the years, we have witnessed the expansion of this roles to include nurse administrators, public health nurses as well as policy makers which has continuously been made possible by redefining and streamlining the curriculum and competencies requirements to best suit the needs of the advance practice nursing (DeNisco, S.M & Barker, A.M, 2024).
After the Nursing Now campaign was launched in 2018, to scale up the universal healthcare coverage(UHC) , the Pan American Health Organization (PAHO) and the World Health Organization (WHO), continued to lobby the governments to implement the advance practice nurses training since nurses had demonstrated great intellectual capabilities in their areas of specialization (Lopes-Junior, 2021). This was a large boost to the nursing profession as it sought to highlight the potential of nurses with an aim of lobbying for them to operate at their full potential and not just confined to the traditional advance practice nursing roles (The Lancet, 2020). More over, this campaign culminated into the recognition of the nurses with the year 2020 being christened “the year of the nurse” (The Lancet, 2020). It is gains as this ones that ultimately led to the World Health Organization publishing the first ever “state of the world’s Nursing Report” and subsequently “the state of the world’s midwifery report” (The Lancet, 2020). Historically, when one spoke of advanced practice nursing, they meant one of four roles; certified registered nurse anesthetist (CRNA), certified nurse mid-wife (CNM), clinical nurse specialist (CNS), or certified nurse practitioner (CNP). In 2004, the American Association of Colleges of Nursing (AACN) defined advanced practice nursing as “Any form of nursing intervention that influences health care outcomes for individuals or populations, including direct care of individual patients, management of care for individuals and populations, administration of nursing and health care organizations, and the development and implementation of health policy” (Denisco & Barker, 2016). Since this definition came out many other nursing roles, such as nurse administrators and public health nurses have also been thought of as advanced practice nursing roles. Whereas advanced practice nursing used to be mainly reserved for clinical practice, it is now branching out into non-clinical roles. With the ever-expanding role of nurses coupled with the push to expand access to and decrease costs of health care there will likely be many new roles developed for nursing practice. These roles will require advanced degrees even if they are not in clinical areas, therefore, they too should be thought of as advanced practice nursing.

I agree with you that nursing practice is any form of nursing intervention that is meant to influence the care outcomes. The nurse practice has been evolving overtime especially with the transformation of the healthcare systems and healthcare policies. In the current healthcare industry where the focus is on the patient, what is commonly referred to as patient centered approach has ensured that the patient is given autonomy to choose on the best care practice that is appropriate and improves the patient’s health condition (American Association of Nurse Practitioners, 2019). This has called upon the nurse training institutions to shift and adapt to the new and emerging trends in healthcare. The advanced nursing role has been the significant part that the Affordable Care Act aims to enhance health promotion measures among the aging US population (Woo et al., 2017). The research findings reveal that the advanced nursing role would be one of the professions at a highest demand by the year 2022.

The advanced practice registered nurse (APRN) has seemed to evolve based on shortages and demands of the population within our healthcare system. Deficits have expanded many nursing roles, leading to the need to define the nursing scope of practice and educational requirements. The traditional four roles of APRN, nurse practitioner (NP), certified nurse-midwife (CNM), certified registered nurse anesthetist (CRNA), and clinical nurse specialist (CNS); have all grown exponentially since the 20th century (DeNisco & Baker, 2016). These four roles all seemed to have stemmed from disparities in our healthcare system, whether it be a shortage of physicians, under-severed rural areas, vulnerable populations, or times of war. A great example of this is the advancements within the CRNA field. CRNAs were initially brought about during World War I due to a physician shortage during wartimes (DeNisco & Baker, 2016). The continuation of wars through United States history gave the CRNA field a strong foothold as a chief provider of anesthetics to the frontline. This expansion of the nursing scope of practice in anesthetics eventually led to credentialing and the American Association of Nurse Anesthetists (AANA) to monitor and regulate this growing field (DeNisco & Baker, 2016). Similarly, the growth and expansion of the other original APRN roles through the end of the 20th and 21st century required the nursing field to define graduate-level educational requirements and each specialty’s nursing role. The transformation of traditional APRN roles has now begun to open doorways for other nontraditional advanced practice nursing areas like public health nursing, clinical nurse leaders, nurse administrators, nursing research, and nurse educators.
DeNisco and Baker do a great job of laying out the ongoing evolution and need of the APRN role in the American healthcare system and the need for more advanced practice educators to sustain the APRN field’s continuous growth (2016). With a growing and aging population and a continuing shortage of physicians, the APRN roles offer a cost-effective solution that can also improve patient satisfaction (DeNisco & Baker, 2016). I will say that this week’s reading has made the decision to continue my education in nursing even more appealing and necessary.

I agree with your statement that the advanced nurse practice numbers have soared high in the 21st century compared by the 20th century. The current Covid-19 pandemic has however, proved that health and wellness is a collective responsibility (Kovac, 2020). In this context both the patient and the nurse are at risk of contracting the disease regardless of age, gender or religious underpinnings hence adequate precautionary measures should be taken. The advanced nursing role has taken up active role in preventive measures of the disease by offering the routine guidelines of hygiene the emerging trends in the spread and severity of the disease. The concept of preventive care is one of the key pillars in the patient protection and well-being. The advanced nurse role requires the nurses to carry out educational programs, seminars and workshops that are meant to help the public on the need of maintaining our bodies for optimal health and reduced chances of hospitalization. The one situation where the advanced nurses’ practitioners have shown resilience is the tackling of the covid-19 pandemic. Countries that embraced strict, firm and taken early hard precautionary measures have known to be safer with very low reported cases in the country (Trnka et al., 2021)

The APRN roles which are the advanced roles include; certified registered nurse anesthetist (CRNA), certified nurse midwife (CNM), clinical nurse specialist (CNS) and certified nurse practitioner CNP). The advanced nurse roles have been traditionally entailing focus on the clinical procedures, advocating for better patient care, being case managers (Hu & Forgeron, 2018). Additionally, ADP is also expected to work in consultation and collaboration with other healthcare professionals. The roles entail a wide scope of practice and ideally, the learning is meant for advancement on the already available information. However, in the current situation the healthcare industry is approaching a new era as emphasized in the affordable care act which we still don’t know what will happen. The affordable care act insists on offering medical care that built on patient centeredness and care that is preventive rather than curative (Courtemanche et al., 2018). This is meant to tackle the rising number of the elderly population that is battling serious chronic illnesses that are associated with old age

The Role of the RN/APRN in Policy Evaluation
Notably, RNs and APRNs have the capacity and the responsibility of influencing the future of healthcare delivery systems. Both RNs and APRNs operate in a framework that holistically values people and creates the need to include them as participants in the policy review. RNs and APRNs are to influence the processes and practice standards that assure the quality of care. For instance, nurses influence a policy that shapes the care effectively in providing care today and in the future (Turale & Kunaviktikul, 2019). The first opportunity for RNs and APRNs to actively participate in a policy review is for nurses to volunteer their time if they are part of members. Secondly, they can run for an office position and internship with the elected officials at a federal, state, or local level.
The challenges that the opportunity may present include a lack of involvement, knowledge, and skill relating to the policy review. Notably, lack of opportunity such as input upon an Adhoc at the various levels includes the lack of experience and poor planning on the process of problem identification (Shariff, 2014). Additionally, the lack of poor enough resources enables the structure to enhance easy participation in the policy review. The lack of enabling structure is considered a barrier in that limited nurse participation in health policy can lead to inadequate representation of nurse leaders.
The two strategies that can help communicate and ensure existence include implementing effective communication that would aid in enhancing knowledge and skill that relates to the policy review. Additionally, implementing good leadership would help ensure effective communication; they will play a role in addressing the issue of lack of opportunity and managing an effective plan that will make the process successful (Hudson et al., 2019). For instance, good leadership will ensure that the RNs and APRNs are experienced in helping the policy review a success.

The American Association of Colleges of Nursing (AACN) defines Advanced Practice Registered Nurse (APRN) as master’s-prepared nurses who provide direct clinical care. This includes the nurse practitioner (NP), certified nurse–midwife (CNM), certified registered nurse anesthetist (CRNA), and clinical nurse specialist (CNS). According to DeNisco, the advanced registered nurse is a nurse with a master’s or doctoral degree recognized by the AACN.
Nurse Anesthetists have practiced for more than 150 years as the main providers of anesthesia care for the military on the front lines during wartime. In 1952, the American Association of Nurse Anesthesiology established the certified registered nurse anesthetist credential to monitor the quality and consistency of nurse anesthetist education. Today, nurse anesthetists autonomously and in collaboration with physicians provide 45 million anesthetics annually.
The first nurse-midwife school was established in 1925 to provide family health, childbearing, and delivery care to women in the Appalachian Mountains due to high maternal and child death rates. Currently, all nurse-midwifery programs are in colleges and universities. Nurse midwives now provide autonomous primary care focusing on pregnancy, childbirth, the postpartum phase, newborn care, family planning, and gynecological care.
The nurse midwife has prescriptive authority in all 50 states and territories and mandatory Medicaid reimbursement status.
The nurse practitioner (NP) role was created in the 1960s to address the shortage of pediatric primary care providers in rural areas. Initially, the training program ranged from 3-12 months. As the NP’s scope and responsibilities changed, the master’s degree became entry level of education required. Today, the AACN recommends a doctoral of nursing practice for NPs. Currently, NPs are the largest group of advanced practice nurses.
The role of the clinical nurse specialist was created in the 1960s to provide advanced care to psychiatric patients. Today, the clinical nurse specialist is able to practice without physician supervision, prescribe medications and durable medical equipment, and order diagnostic tests in some states.
The level of education and training for entry as an advanced practice nurse continues to increase as the scope of practice and responsibilities also increases. The population is aging with more co-morbidities. The predicted physician shortage signals an impending lack of care for patients with complex care needs. Advanced practice nurses must be educated, supported, and empowered to continue to step in and fill the gap to meet the demands of the patients.
