Introduction to Interprofessional Collaboration

Interprofessional collaboration in the healthcare industry is a vital component for delivering top-notch patient care. By bringing together individuals from various professional backgrounds and specialties, healthcare providers are able to offer a comprehensive and integrated approach to patient care (Karam et al., 2021). This type of collaboration requires a team-oriented approach that brings together the diverse perspectives, skills, and expertise of multiple healthcare professionals. This results in the development of a shared goal, which is the well-being of the patient. Through Interprofessional collaboration, healthcare providers are able to identify and address the complex and multi-faceted needs of patients. They can work together to develop a personalized care plan that takes into account the patient’s medical history, current health status, and any ongoing health concerns. This leads to improved patient outcomes and a higher level of patient satisfaction. In this paper, I will discuss my experience of Interprofessional collaboration, and how it could have been better approached.

Scenario

Interprofessional collaboration can occur in various settings, including hospitals, clinics, and long-term care facilities. In my scenario, I am part of a healthcare team consisting of a doctor, nurse, and physical therapist, working together to treat a patient with a musculoskeletal injury. The doctor plays a crucial role in this process, as they are responsible for diagnosing the injury. They will perform a physical examination and may order diagnostic tests, such as X-rays or MRI scans, to determine the extent of the injury and its underlying causes. Based on their findings, the doctor will then make a diagnosis and develop a treatment plan. The nurse, on the other hand, plays an important role in managing the patient’s pain and medication. They will work closely with the doctor to determine the best course of action for managing the patient’s pain, whether it be through over-the-counter medications, prescription pain relievers, or alternative therapies. The nurse will also monitor the patient’s response to the pain management plan, making any necessary adjustments to ensure that the patient is as comfortable as possible.

Finally, the physical therapist plays a key role in helping the patient recover from their injury. They will work with the doctor and nurse to develop a rehabilitation plan that addresses the specific needs of the patient. This may include a combination of exercises, stretches, and other physical therapies aimed at improving the patient’s strength, mobility, and overall function.

Establishing Clear Goals and Expectations

One of the first steps in improving Interprofessional collaboration is to establish clear goals and expectations (Banister et al., 2020). In my scenario, the healthcare team should have set clear and specific goals for the patient’s treatment and recovery, and aligned their individual plans to achieve those goals. This could have been done through regular team meetings and clear communication channels, where each member of the team could provide input and discuss their plans. By having clear goals, the healthcare team would have a shared understanding of what needs to be achieved, and each member would know what is expected of them.

Encouraging Open Communication

Open and effective communication is also crucial in Interprofessional collaboration. In my scenario, the healthcare team should regularly meet and have open and honest communication about the patient’s progress and any challenges that arise. This would have allowed the team to work together to find solutions to any obstacles and to make adjustments to the patient’s treatment plan as needed. For example, if the physical therapist noticed that the patient was having difficulty with a specific exercise, they could have communicated this to the doctor and nurse, and worked together to find a different approach that would be more effective.

Sharing Expertise and Knowledge

Sharing expertise and knowledge is also important in Interprofessional collaboration. Each member of the healthcare team brings unique expertise and knowledge to the table, and it is essential that this expertise is shared to improve patient outcomes (McCave et al., 2019). In my scenario, the doctor, nurse, and physical therapist should have discussed their individual plans and strategies for the patient’s treatment, and shared any insights or suggestions that could have been helpful. By sharing their knowledge and expertise, the healthcare team would have been able to provide a more comprehensive and well-rounded approach to the patient’s treatment.

Fostering a Team Mentality

Finally, fostering a team mentality is essential in Interprofessional collaboration. The healthcare team should work together as a unit, recognizing the importance of each member’s role and contributions to the patient’s overall well-being. In my scenario, the doctor, nurse, and physical therapist should have worked together to support each other and to ensure that the patient received the best possible care. For example, if the nurse noticed that the physical therapist was feeling overwhelmed, they could have offered support and helped with some of their tasks, so that the physical therapist could focus on the patient’s rehabilitation.

Conclusion

In conclusion, Interprofessional collaboration is crucial in providing high-quality patient care. By establishing clear goals and expectations, encouraging open communication, sharing expertise and knowledge, and fostering a team mentality, the healthcare team in my hypothetical scenario could have provided better patient care and achieved better outcomes. By working together as a unit, healthcare professionals can provide a more comprehensive and well-rounded approach to patient treatment, and ensure that the patient’s well-being is at the forefront of all decision-making.

Interdisciplinary Collaboration Experience

Background of the Problem

In my former healthcare organization, we noticed an increased incidence of adverse drug reactions (ADRs) among geriatric patients with comorbid chronic illnesses. ADRs had become a major cause of hospital admissions and emergency department visits in the facility.  The pharmacy department sought to identify the cause of the increased cause of ADRs in these patients and established that most of the patients who presented with ADRs were on multiple medications mostly 5-9 medications. The clinical pharmacists also established that some of the drugs the patients were taking caused harmful drug-drug interactions. Furthermore, patients reported that every time they were discharged or consulted a different specialist, they were added new medications, contributing to the polypharmacy.

The Plan

Based on the findings from the clinical pharmacists, the hospital created a pharmacist-led medication reconciliation program at admission and discharge. The program was designed to take an interdisciplinary collaboration approach with the following healthcare professionals:

  • One clinical nurse leader (CNL) – The CNL was in charge of the team.
  • Three Clinical pharmacists
  • Four Clinicians including two physicians and two Nurse practitioners (NPs)
  • Five Registered nurses (RNs).
  • A medication reconciliation using a transition of care interdisciplinary team was proposed to minimize the number of medications with errors during hospital admission, discharge, and follow-up visit (Kreckman et al., 2018). This would identify unnecessary medications and those with interactions, thus mitigating ADRs caused by drug-drug interactions.

The interdisciplinary approach was successful since each member of the team was assigned clearly outlined laws preventing duplication of roles or confusion.

The interdisciplinary medication reconciliation process occurred as follows:

  • During the admission of each geriatric patient, RNs carried out a two-step verification of prescribed medications, updated the electronic medical record, and informed the clinician to sign off on the list.
  • At discharge, a patient’s medications were reconciled in the ward by the RNs, and the medication records were forwarded to a patient’s primary care provider.
  • During follow-up visits, an RN and physician verified a patient’s medication list.
  • RNs commenced the medication reconciliation at admission and reviewed the medication list within 24 hours of a patient’s discharge.
  • The clinical pharmacists evaluated the list presented by the RNs, rectified medication issues like drug duplication and omissions, and proposed evidence-based therapies (Burgess et al., 2021).
  • The clinical pharmacists also informed the clinicians of medication issues identified during admission and discharge.

How Poor Collaboration Can Result In Inefficient Management of Human and Financial Resources

  • Poor interprofessional collaboration can lead to ineffective management of human and financial resources. Poor collaboration especially during handovers increases the probability of medical errors occurring (Stewart, 2018). This leads to suboptimal patient care that causes poor patient outcomes and resultant high medical and operational costs.
  • Inefficient management of financial resources occurs when there is a lack of procedures or uniformity in procedures used (Stewart, 2018).
  • Poor collaboration is associated with increased staff burnout and work-related stress. This leads to low motivation, increased work absenteeism, and high staff turnover. Low motivation and increased work absenteeism is a sign of inefficient management of the human resource.

Best-Practice Leadership Strategies

Effective leadership strategies are fundamental for the success of an interdisciplinary team’s ability to attain its goals. These strategies include:

  • The leader should have a clear vision and communicate it: There needs to be a clear vision, role, and purpose for the team (Schot et al., 2020). The interdisciplinary team in our facility had a team leader who shared the vision of the medication reconciliation program. This steered the direction of the team and helped it establish appropriate referral criteria for patients.
  • Good communication: The leader should communicate to the team members in a way that they feel they can listen and express their views, including discussing and resolving challenges within the team.
  • The leader should also promote role interdependence in the team while respecting individual roles and autonomy (Schot et al., 2020).
  • The leader should foster a team culture and interdisciplinary atmosphere of trust where team members’ contributions are valued and the consensus is promoted.

Best-Practice Interdisciplinary Collaboration Strategies

Effective interdisciplinary collaboration strategies are essential in helping the team attain its goals and collaborate more effectively.  The strategies include

  • Respecting and understanding members’ roles: There is a need for each member to respect and understand the roles of other team members, as well as, understand the limitations and boundaries of each role (Buljac-Samardzic et al., 2020). In our interdisciplinary team, the team member understood each other’s roles and how the different roles had the potential to affect patients.
  • The team members should identify a leader who sets up a clear direction and vision for the team while listening, supporting, and guiding the team members.
  • The leader should utilize communication strategies that promote intra-team communication, collaborative decision-making, and effective team processes (Burgess et al., 2021).
