Delivering excellent quality of service and helping vast amounts of patients and making a difference, I believe, is every health care professional vision. Unfortunately, challenges affecting the quality of care still arise. As a new care coordinator in the surgical area, a subject requiring improvement in the surgical setting is the need for surgical site infection prevention. Although the strict implementation of protocols and required guidelines are set by state or federal regulatory agencies, surgical site infections still remain an issue leading to post-op patients complications and sometimes even death. Therefore, the need for competent nursing practice and clinical expertise is absolutely essential to effectively support the change process on improving outcomes and bring overall success. Lippitt’s seven steps change theory could definitely be utilized in helping with this particular issue. The emphasis is on using practical communication skills, outlining problem-solving approaches, and establishing feedback. This method uses a democratic style of leadership concerned to identify or diagnose the problem, incorporating a well-detailed plan. The change theory adopts a similar model to the nursing process involving systematic phases of assessment, planning, implementation, and evaluation (Mitchell, 2013).

For this reason, the concept can be connected to surgical site infection prevention. The change theory can be used to revise the existing surgical site prevention methods, outlining the problems, and use systematic change phase strategies to prevent surgical site infections, ensure patient safety and positive outcome. I think this model is beneficial to patient outcomes and can be applied in this area of specialty as surgical site infection continues to be a key point of concern. For example, in the assessment phase, risk factors associated with surgical site infections will be recognized and addressed. The following steps are to propose the action necessary to implicit change and reevaluate the implemented plan, to determine if the goal was reached. This theory is aimed to help nursing professionals in daily practice increasing awareness in their decision making, recognizing and eliminating potential problems. Application of this theory may be especially useful when newly evidenced-based research suggests changes to the current practice that should be adopted as a best practice to improve patient safety and outcome.

An ineffective leadership trait that I’ve witnessed over the course of my career is micromanaging. This notion of if you want something done correctly you have to do it yourself is damaging, it causes unhappy employees, fatigue, and burnout. Micromanagers also negatively impact the morale of the department because it says, “I don’t really trust that you know what you’re doing” so these leaders constantly second guess the abilities of their staff. Delegation is important because everyone has a role to play and when everyone plays their role correctly, processes run smoother. A leadership role is meant to inspire and lead not to control (Ringer, 2019). Employees need the room to develop their skillset and solutions to problem that arise. I will lead by example because how will I teach autonomy to patients and other nurses if I do not practice the ethic of Autonomy in my own nursing career. Strategies I may use as a future leader include active listening. Listening to suggestions from employees speaks magnitudes because the impression is they are heard and respected by delivering what is requested for a better work environment. Recognizing when employees are exceptional is another approach to good leadership, yes, correcting when it warrants but also celebrating. Communicating valuable information as well as creating a safe space for communication amongst employees is also vital. When leaders fall short so does the team, everyone suffers.

Role of an Advance Registered Practice Nurse

The nursing profession is no exception to change. To have an effective and functional healthcare system, the roles and responsibilities of advanced nurses and other health care workers must be redesigned well to meet the ever changing population. The scope of an advanced registered practice nurse has changed overtime. The traditional level of nursing practice is unable to meet the needs of the current increased population (DHANDAPANI & L G, 2020). The United States of America has almost four million registered nurses in different cadres and specializations. This paper helps determine the transformational journey of the nursing profession through the 20^th and 21^st centuries.

There exists a challenge in the management of chronic diseases in the United States, due to the new healthcare laws. A personal worldview entails the principles that guide one’s decisions and actions in life. A worldview allows one to make sense of their life and the world and determine how they perceive reality (Butts & Karen, 2018). Additional, these principles guide ethical and moral beliefs and actions while enabling one to integrate different aspects of religion, culture and spiritual wellbeing and responding meaningfully to challenges and opportunities in life. The essence of this reflection paper is to describe my worldview and the nursing theory that impacts my practice.

Personal Worldview

Nursing in my worldview, which is Hindu, is a calling to serve humanity based on nature and the need to do good in the world. Hindu worldview believes in reincarnation of the soul to expand Brahman’s work. Each soul is pure and held back from its reabsorption into Brahman by the karmic effects of one’s actions (Thompson, 2017). As a practicing Hindu, I believe in Karma which entails doing good and being faithful to “dharma” or my responsibilities in life, especially in my family and community. Karma teaches self-control and non-violence as high values and moral virtues that none should depart from in their duties and obligations. Hinduism places importance on dharma or the proper way of life (Brown, 2020). The implication is that as a practicing Hindu and nurse, I must ensure that I take my duties seriously and live the proper way so that karmic effects lead to the reincarnation of my soul in the next life as essential person.

Nursing involves the application of acquired knowledge and skills in care to relieve patients from pain, suffering and distress. Nursing is about taking responsibility for continual growth in human connection, developing character and showing compassion (Butts & Karen, 2018). As highlighted by my worldview, especially the concept of karma, nurses need to do good and demonstrate positive values as they handle the broken parts of their patients’ lives. As a nurse, one must address any personal weakness and work towards individual changes and growth to offer effective care to patients.

Being kind to patients, caring for them, having an ear for them and understanding their pain and demonstrating concern, genuine love and compassion lead to more healing than the use of medications and other interventions. Personal growth and having principles based on one’s worldview are essential in provision of holistic care for those who require help by developing connection, trusting relationships and respect to individuals’ beliefs in healthcare and offering hope to enhance quality of life of the suffering patients and their families.

Nursing Theory or Model

Nursing is about caring for humanity and communities with the aim of serving them to attain better and quality care. As nurses, we care for patients and believe that we have a duty to ensure that they get better. Therefore, the most appropriate nursing model based on my personal philosophy and worldview is Jean Watson’s caring model. Watson’s model is founded on the principle that by virtue of taking care of a patient, the nurse gets into their life space and spiritually to associate with their condition. Consequently, the nurse develops and attachment with the patient at deeper spiritual and emotional levels (Grinspun & Bajnok, 2018). The model centers on holistic approach to care than treatment plan and pathology. Watson’s model asserts that while a disease can be cured, illness may persist as health is not attained without effective caring.

The primary similarity between Watson’s model and my personal worldview in nursing is that the two advocate for better and positive care which does not focus on elimination of disease alone but also addressing the needs of a patient in a holistic manner (Wei et al., 2019). The two perspectives advocate for provision of health care by getting into the deeper levels of connection with the patient at the spiritual level. My worldview, especially karma, advocates for doing good since I will only live to account for my deeds in the next life. Therefore, I must develop therapeutic interactions and relationships that lead to hope, trust and propagation of positive beliefs towards healing and wellness.

The theory reinforces a holistic approach to care through its main concepts that include carative factors, transpersonal caring relationship and the caring occasion. The carative components offer specific guidelines which align with the core part of caring and promotion of quality care. A transpersonal relationship involves the demonstration of a positive attitude towards care and the integration of religious and social components (Sitzman, 2018). The caring occasion illustrates love-focused interaction between individuals which involves authentic sharing of experiences that widens one’s perspective in health practice. As such, through these aspects, the nursing practice profession attains resilience in provision of patient care and reinforces appropriate models in care delivery.

Example in Practice

Mental health cases are usually ignored in the community and many would think that at this coronavirus disease (Covid-19) pandemic, state and federal health agencies would prioritize such needs. Many individuals with mental health issues are neglected and even those facing the increased disruptions from the pandemic have not been spared the neglect and limited access to healthcare services (Pfefferbaum & North, 2020). I have encountered many patients who come to our facility and seek medical attention but a closer analysis and assessment demonstrate that many are in need of mental health services due to the effects of Covid-19 pandemic. I recall this particular patient who was neglected by providers in our facility after they realized that perhaps he was suffering from depression and anxiety as a result of losing his job and one of his friends to Covid-19 disease. The negative attitude that the patient received from nurses and other provider was disheartening if not outright, uncalled for since he required attention.

In this case, my worldview and the nursing model helped to do good by resolving the issue when I established his needs. I called a psychiatrist after the normal assessment and the patient started attending sessions to help him deal with his problems. He showed his gratitude one day when he came to thank me. I believe I did my part as expected by karma.

Future Practice

My worldview in nursing practice and the nursing theory demonstrate the need for nurses to center on patient-focused care and enhance their understanding of their needs. In this case, these models will help to develop a patient-centered and evidence-based practice philosophy guided principles learned from both. I will strive to offer better care and consider a patient as a critical part of my nursing practice. I will enhance my proficiency in cultural care and ensure that I get knowledge and skills to enhance quality care provision. Watson’s model focuses on maintenance of a healing environment that centers on humanistic approach of care (Watson & Woodward, 2020). Therefore, this approach aligns with my personal worldview and would be fundamental in helping become a better nurse for patients.

Nurses dedicate their expertise based on their specialty to serving different populations. The existing health disparities and the need to meet the recommendations of the report on nursing by the National Academies of Science, Medicine, and Engineering on the national path for the future of nursing entail enhancing access to health services for vulnerable populations (Wakefield et al., 2021). Disparities occur due to differences in the incidence, prevalence, mortality and disease burden among different populations.

Rational minorities, particularly those from low income and limited English proficiency backgrounds encounter multiple health associated disparities like barriers to health, limited resources and accessibility as well as quality of care (Braveman et al., 2019). As an advanced practice registered nurse (APRN) in nursing leadership, I want to impact this population through health promotion and disease prevention approaches. Unaddressed cultural variances between nurses and patients can lead to difficulties in diagnosing and non-compliance by patients. Cultural competent care implores nurses to have a holistic understanding of patients; which includes their cultural values and practices to attain better patient outcomes. Nurse leadership ensures that all nurses understand and appreciate the different facets of cultural competent care through effective training and dissemination of information and interventions. Disease prevention and health promotion implore nurses to develop better interventions that incorporate patient values and preferences.

Diversity and global perspectives of care are essential in understanding health equity associated with this population. Diversity implores nurses and other providers to accommodate different practices and values that can enhance care outcomes while global perspectives reflect efforts to consider different components that impact care provision and acceptance among peoples and cultural groups (DeNisco, 2019). Therefore, health equity for this population requires understanding the cultural, the perspectives, and even financial barriers that hinder their use of healthcare resources.  Health disparities are mainly systemic and may be as a result of intentional or unintentional discrimination. These aspects also demonstrate the variance in ideas for different users of the healthcare products among different populations.

An opportunity for improvement would be to increase the quality of the interpersonal relationships between the patient and the nurse. Improving this nurse-patient relationship would enhance the quality of care and health status of the patient. In assessing the current environment, in my opinion, Peplau’s Theory of Interpersonal Relations would be the most effective. Comprehending this theory and the connectedness that arises from this theory provides a framework for the nurse-patient relationship (Wasaya, F., Shah, Q., Shaheen, A., Carrol, K., 2021). Peplau’s theory emphasizes the patient experience in the nurse-patient relationship. Peplau’s theory of a nurse-client relationship has 4 phases orientation, identification, exploitation, and resolution. The orientation phase involves engaging the patient with treatment options, supplying information, and answering questions. The identification phase is a collaboration between the patient and the nurse allowing the expression of the patient’s feelings and a sense of strength is developed. The exploitation phase allows the patient to make full use of offered services. The resolution phase is when the patient has resolved the issue and no longer needs contact with healthcare providers.

In my current working environment in an ambulatory care center for the Veterans Administration (VA), I have identified a weakness in the patient-nurse relationship. Contributing factors include the difficulty that comes with this specific patient population. However, it seems that many of the nurses are lacking in developing strong patient-nurse relationships, and I see how this affects the care the patients receive. This deficit limits the quality of care the patient receives and has a negative impact on patient outcomes. An example of this is when one of the nurses asked why I give patients my extension. My response was so they can contact me, that’s why I am here to assist the patients. I strongly believe improving interpersonal relations with our patients would benefit the patients and also the nurse.

A worldview denotes ways in which individuals translate the world around and develop a philosophy in their approach to care. A worldview comprises of the ideas and beliefs that influence one’s thinking about different concepts that include cultural, religious and spiritual aspects. Nurses must develop their worldviews for effective care delivery through interactions with patients from diverse backgrounds (DeNisco & Barker, 2017). Nurses are expected to offer care that meets cultural, spiritual, social and religious needs of patients. The purpose of this essay is to reflect on my current and future practice and the ways that worldview and nursing model will influence these aspects.

Personal Worldview

My personal worldview is shaped by my experience and interactions with a multidimensional culture of care that values respect for personal views of other individuals based on their experiences and perspectives. I believe that as nurses, irrespective of one’s specialty, we are called to serve, either through a religious understanding and perspective, or through one’s philosophical underpinnings. Nursing encompasses taking responsibility for continual growth in humanistic interactions, development of character and showing compassion to sick and suffering patients with different conditions. I believe that nurses are genuinely passionate about caring for patients and are called to serve. The implication is that as a nurse, one must develop spiritual love and caring as qualities to build therapeutic relationships with patients and improve care delivery. Spiritual care emanates from the heart and nurses should always focus on enhancing patient care through a holistic approach that include physical, mental, social and spiritual components (DeNisco & Barker, 2017). Respect for life is a core component of my cultural care worldview and I believe that God places nurses in unique caring positions to demonstrate his love for humanity. Spirituality is an essential component in many world cultures and by believing in a higher power, nurses do not consider themselves as the only responsible for better patient outcomes. Cultural sensitivity is a critical aspect of care delivery as it implies that nurses understand the differences that exist among patients and their families and how to treat each patient in a unique manner for better outcomes. It is also important to realize that these differences are sources of strength and can help define one’s nursing practice philosophy.

Theory Aligning with My Worldview and Its Reinforcement

As a registered nurse who wishes to specialize in informatics, I believe that diversity and universality are elements shaping the healthcare industry and essential aspects of attaining patient-centered care. Imperatively, the specific nursing model that aligns with my worldview is the Leininger’s theory of culture care diversity. Also called the transcultural nursing theory (TCN), the model’s concepts and propositions require nurse practitioners to use culturally respectful and congruent nursing care based on patient’s beliefs, values and cultural practice (McFarland & Wehbe-Alamah, 2019). The model requires nurses to use culture-specific nursing care that is not only holistic but also recognizes the patient’s values and their general worldview.

The similarities between the model and my worldview are evident. Both the model and my worldview consider respect for patient’s cultural beliefs and opinions. The theory and my worldview also appreciate the respecting, accepting and integrating patient’s cultural aspects and practices into care plans does not affect the quality of care offered. The implication is that the integration enriches care quality by making it culturally significant and sensitive to the needs of the patient and their family. My personal worldview focuses on empowering patients through acceptance and appreciation of their cultural preferences (Sagar & Sagar, 2018).  Therefore, Leininger’s culture care theory reinforces this by emphasizing a patient advocacy philosophy and a patient-centered approach to nursing care. The implication is that both the theory and personal worldview emphasize the need to uphold the bioethical principle of independence or autonomy.

The philosophy entails understanding and appreciating the uniqueness of each patient’s cultural beliefs and values on issues like diet, traditional medical interventions, healing through faith, and the place of prayers among others. At the core of these similarities is the advocacy of culturally congruent nursing care for each patient at individual levels with no prejudice or biased judgment and stereotypes (McFarland & Wehbe-Alamah, 2019). The theory reinforces my perspective as it implies that culturally competent and acceptable nursing care does not require one to change their worldviews. Contrary, it implies that nurses must appreciate, accept and embrace each patient and their uniqueness and device evidence-based practice strategies to develop appropriate care plans for optimal outcomes.

Example in the Current Practice

It is not always easy and certain to implement what the conceptual propositions of theories advance in clinical settings. However, in situations where there is convergence between personal worldview and the nursing theory’s suggestions, the incorporation of such evidence becomes easy and effective. The two aspects become essential in changing a current or past practice that may not align with the ideology propagated by both worldview and theory. The present practice of rejecting all types of traditional or alternative and complementary medical interventions because of lack of evidence on their effectiveness based on empirical research is a practice that rejects the incorporation of patient’s opinions and views (Wilson et al., 2018). Nursing like other medical and healthcare disciplines only recognize the use of therapies and interventions supported by research because of the concept of evidence-based practice. It implies that for an intervention to be deployed in managing a patient’s condition, it must be based on strong empirical evidence that supports its effectiveness.

Contrary, most of the culturally accepted remedies and alternative medical interventions are not backed by empirical evidence for their efficacy. The issue is that rejecting these interventions even in instances where the patient believes in their working and effectiveness negates the concept of culturally component and sensitive care. Therefore, my worldview and the transcultural care model would help me to correct this problem by prescribing acceptance and education for practitioners and assisting the patient understand the basis of the present position. As a nurse, one must no judge and dismiss a patient’s preferences as inconsequently and with not effective outcomes (Wilson et al., 2018). Instead, the nurse should accept the preferences and use tactful ways in a culturally competent manner to educate the patient on the facts concerning the use of both alternative medicines and conventional approaches.

Influence on Future Practice

Nurses must recognize that their personal worldviews should not always be congruent to patients’ because of different cultural and social reasons. Imperatively, the nurse must be culturally competent and provide culturally congruent care by adapting their views and perceptions to a patient’s worldview. Therefore, having a multidimensional culture of care approach and getting support from Leininger’s model will be critical in my future nursing role and practice (McFarland & Wehbe-Alamah, 2019). As a future nurse informaticist, I should incorporate cultural dimensions in my role for effective interpretation of data and making effective decisions that will enhance care delivery. As an informatics nurse, integrating cultural competence in all aspects of data analysis will help in effective decision making and developing reports for easy implementation in practice for other providers. Through this approach, I will enrich my future career as an informatics nurse by ensuring that it is culturally sensitive and congruent to patient’s needs.

Nurses play crucial roles in the promotion of optimum health and wellbeing of the populations that they serve. They explore the available best practices that can be utilized to achieve care outcomes such as safety, quality, and efficiency. In doing so, worldviews guide nurses in making their decisions. Worldviews encompass the philosophies that guide one’s decisions in their daily lives. Therefore, this essay examines my worldview as a nurse, an appropriate theory that aligns with it, and its effect on my future practice.

My Personal Worldview

My worldview as a nurse focuses on the provision of holistic nursing care to patients. I believe that nursing care should not only focus on the patient’s health problems but also the factors that influence their experience with a disease. The care should aim at identifying the different factors that influence patient’s adherence to the treatment plan and their ability to transform their lifestyles and behaviors (Murgia et al., 2020). Holistic nursing care empowers patients to take responsibility for their health needs, hence, sustained outcomes. I also believe that patients play a vital role in the realization of optimum care outcomes. Patients should be involved in assessing, planning, implementing, monitoring, and evaluating their care outcomes (Hertz & Santy-Tomlinson, 2018). Patient involvement in the care process contributes to their satisfaction as well as adherence to the treatment plans.

Part of the holistic care in my worldview encompasses respecting patients’ cultural values, beliefs, and practices. Nurses should make care decisions that align with the diverse cultural values and practices of their patients. They should possess the cultural competencies to provide care that addresses the prioritized and potential needs of their patients (Frisch & Rabinowitsch, 2019). I also believe that nursing care should also consider the religious beliefs and practices of the patients. Nurses should assess the meaning of religious beliefs and practices to patients and their recovery process (Hertz & Santy-Tomlinson, 2018). Aspects such as having clergies attend to patients’ spiritual needs should be adopted to ensure holism in nursing care.

My worldview also focuses on the utilization of evidence-based interventions to ensure optimum care outcomes. I believe that nurses should make their decisions based on best clinical and research evidence, expertise knowledge and the consideration of patient values (Chien, 2019). I also consider patients as unique beings with the abilities to meet their self-care needs. Nurses play the roles of assisting them to meet their needs when unable in times of illnesses. Nurses should empower patients to develop self-care abilities and meet their daily needs. Lastly, I believe that nursing is a caring profession that relies mainly on interprofessional collaboration (Camargo et al., 2018). Nurses should collaborate with other healthcare providers to meet the diverse needs of their patients.

Nursing Theory

Dorothea Orem’s theory of self-care deficit aligns the most with my philosophy of nursing. According to Orem, nurses’ roles in the patient care is an act of assisting them in the management and provision of self-care to ensure the maintenance or improvement in human functioning. Nursing care roles aims at helping patients to perform self-care, maintain their lives, wellbeing, and health (Ali, 2018). Orem considered that nurses help patients in times of illness to connect among themselves as well as their environment. In their role, nurses help patients to identify their self-care deficits and work towards meeting them independently for their optimum health and functioning. Orem’s theory also asserted the need for legitimate relationship between nurses and patients (Hartweg & Metcalfe, 2022). Nurses develop therapeutic relationships with their patients with the aim of influencing their behaviors and promoting healthy decisions that contribute to their health.

Orem’s theory of self-care deficit shares some similarities with my worldview. First, I believe that patients are independent agencies that have the will and power to meet their health needs. Illness affects these abilities, increasing the need for nurses to assist patients achieve the independence they desire. The other similarity lies in the focus on the provision of holistic patient care. Orem recognized the need for nursing care to focus on aspects that include patient, health, environment, and nursing. This focus is important in ensuring that nurses provide care that is multifaceted in nature (Hartweg & Metcalfe, 2022). For example, besides providing care that aims at the identified illness, nurses should ensure that the environment and nurse-patient relationship facilitate the recovery process.

Orem’s theory enforces my approach to care in some ways. First, it helps me identify the prioritized and potential self-care deficits for my patients. This focus helps in the development of patient-centered and appropriate plans that promote recovery and patient’s wellbeing. Orem’s theory also informs my approach to helping others to achieve their self-care needs. For example, it informs my use of approaches such as acting and doing for others, guiding, supporting, teaching, and providing an environment that supports development for those I care in my practice (Yip, 2021). Lastly, Orem’s theory informs my approach to nursing process. I incorporate the theory into the different phases of nursing process such as assessment, planning, implementation, monitoring, and evaluation (Helou & Zumstein-Shaha, n.d.). For example, I evaluate the developed nursing care plans based on the achieved self-care abilities by patients.

Example

I apply my worldview in caring for my patients in my practice. An example of the way in which I recently applied it was when caring for a patient practicing Christianity. The patient believed that his health would improve if prayed by a clergy. As a result, I organized for a clergy to visit and have a prayer with the patient. The patient was satisfied with the care given and was discharged afterwards. He believed that besides treatment, prayers helped him recover from his illness.

Effect of My Worldview and Theory on My Future Practice

My worldview and Orem’s theory will assist me in developing my future practice in some ways. First, my worldview and theory will inform my research in nursing. I will utilize the concepts of Orem’s theory in investigating the different issues that affect nursing and nursing practice (Gligor & DOMNARI, 2020). My philosophy will also inform my personal and professional development initiatives as a nurse. In this case, I will reflect on my professional experiences and explore opportunities for growth with the aim of improving my practice as a nurse. The theory and worldview will also strengthen my understanding of patient needs and the delivery of patient-centered care (Hartweg & Metcalfe, 2022). Nursing theory will help me learn about new evidence-based interventions guided by theory that I can utilize to enhance patient care outcomes.
