Effective communication among interprofessional teams is essential when taking care of complex patients. As technology advances, new and different ways to communicate between providers, patients, and other health care organizations (HCO) are evolving, and many of these different systems are working together to improve quality patient care. One specific communication technology is the electronic medical record (EMR). The EMR allows multiple healthcare professionals to access a patient’s chart at the same time to document or review. Providers can also share their encounters easily with other providers that have consulted a specific specialty. For example, if a primary care provider (PCP) consults cardiology, the cardiologist can route their encounter back to the PCP once the patient has been seen instead of the PCP searching through the chart. The cardiologist encounter is there in the chart for all to see at any time, but routing it back to the inbox of the PCP notifies them it is complete.
The information in the EHR is streamlined into different categories such as encounters, lab results, and imaging to name a few. Advancements have been made to improve EMR use such as dictating notes instead of typing them and use of smart phrases to increase efficiency when completing notes. The EMR also works with other advancements including apps that patients can download on their smartphone to access certain areas of their chart, and providers also can download apps on their encrypted smartphones to access the EMR on the go. At the organization where I currently work, many providers and nurses also have encrypted laptops to easily access the EMR. This saves time searching for a computer to use. It also allows real-time electronic data entry of clinical information and reduces delayed entry of notes into the EMR making patient data available to other professionals (Bardach et al., 2017). Other HCO can also share information through the EHR allowing interprofessional communication faster than waiting for health records to be faxed or mailed reducing the wait time and the potential of being lost, sitting on the desk of an administrative assistant, or at another location of the HCO.
