These evaluation programs are also important in improving the program and making informed-decision on an issue under investigation (Rohatinsky et al., 2020). Evaluation design allows for determining the effectiveness of projects and offering informed-decision on the changes that can be made to ensure that the desired outcomes are met. Therefore, the purpose of this assignment is to develop an evaluation plan on the identified clinical problem of the gap between a senior-year nursing student and a new nurse.

            The proposed project seeks to identify the clinical problem of the gap between a senior-year nursing student and a new nurse. This gap has been widening in the recent past because most healthcare professionals lack a program to mentor new nurses into practice. The effect is evident in compromised patient safety and healthcare quality. The expected outcome from this project includes bridging of the gap between senior nursing students and new nurse. This will be reflected in improved patient care, reduced readmission rates, improved teamwork, and high job satisfaction among new nurses. Poor patient care has been an issue linked to inadequate skills among professionals to devise better methods of patient care. Improved patient care would as well result in low readmission rates that further relay more information on the improved quality of the healthcare delivery (Rohatinsky et al., 2020). The evaluation of these outcomes would be significant in understanding the effect of mentorship in improving the clinical outcome among new nurses.

            Recording the readmission rate after undertaking the intervention in the mentorship program in a healthcare facility would be significant in measuring the impact of the intervention in improving the desired outcome. The outcome from the readmission rates would be effective in establishing a framework that can be used to achieve quality, safety, and experience of care (Albarqouni et al., 2018). The outcome of reduced readmission would show that the care offered to patients has improved, and nurses have gained addition and effective skills in attaining the unique needs of the patients at the facility.

Data Collection Tool and Data Collection Method

            Electronic healthcare records (EHRs)will be the primary data source for the evaluation plan. This makes the EHRs a significant data source in the evaluation plan. The data from this system will show if the proposed intervention is effective and can meet the desired patient outcome by improving the mentorship of new nurses. Again, the EHRs effectively capture the most accurate patient data as they are used in the daily monitoring of patients in an institution (Brownson et al., 2018). This makes the tool significant in developing an evaluation plan.  

            Chart audits will also be used in this project. Chart audits done internally are used to measure activities that aid in supporting an organization to achieve its main objectives, the evaluation of care quality, and the assessment of staff performance (Terweeb et al., 2018). This tool will be designed to have a series of questions that seeks to clarify whether the mentorship programs are important in assessing the nurse’s skills. The outcome will be measured in percentages, and the data will involve determining consistency in the data collected.

            This project will use an interval test. This statistical test is selected because it can determine whether the outcomes on blood pressure vary within the two interventions. The trend recorded on patient outcomes before the intervention will be compared to the trend noted after (Terwee et al., 2018). This comparison would provide the most effective conclusion that will either support or decline the proposed project.

Part 2: Discussion


            Nurses have an essential role to play in the intervention program. The process would entail engaging the nurses in establishing the program. This process will entail increasing the flexibility of the nurses to allow them to have time with their mentors during the process. Maintaining the intervention after implementation will be achieved by engaging nurse leaders and other frontline workers because they are the main drivers of this project. The healthcare providers must receive satisfactory training on implementing and sustaining the proposed solution (Albarqouni et al., 2018). The unity among nurses would be effective in developing a mentorship plan that would be significant in meeting the desired outcome.

            The intervention plan would be significant in developing the relationship between the senior and new nurses at the facility. Indeed, the senior nurses would get the opportunity to sharpen the skills of the new nurses, who would be expected to drive the facility towards meeting its positive outcome on patient safety and quality (Rohatinsky et al., 2020). The organization and the new nurses would gain from the intervention as it would improve interprofessional collaboration. The improved care would be one of the significant outcomes the facility would expect from the process.

Future Steps

            The current project would be significant in enlightening the new nurses on the new technology used that can improve the desired outcome. The process would result in the development of skilled young nurses with the desire to meet the desired healthcare outcome (Chan et al., 2020). In doing so, the new nurses would also increase in number and could effectively attend to a large population of patients. The impact would be felt in the entire healthcare system as all the nurses would work towards a common goal of improving patient care and safety.

The goal for this is to finalize the outcomes that your plan is seeking to achieve and to create a plan to evaluate the degree to which those outcomes were achieved after your intervention plan has been implemented. This will allow you and your care setting to determine the degree to which the plan was successful in addressing the identified need of your target population and setting.

You will also discuss ways in which your role allows you to lead change and drive quality improvement, and to potentially improve the project in the future. In addition, you will reflect on how the project will leave you better prepared for success in other aspects of your current and future career. Provide enough detail so that the faculty member assessing your implementation plan design and discussion will be able to provide substantive feedback that you will be able to incorporate into the final draft of your project.
