The most common sites for metastasis on J.C are the liver, lung, and peritoneum. Ayres Pereira & Chio (2019) explain that in human Pancreatic Ductal Adenocarcinoma (PDA), the liver is the most common site of metastasis and occurs in over 60% of the patients, followed by lung and peritoneum metastasis accounting for about 30% of the patients.

Tumor cell markers are substances produced by normal or neoplastic tissue and often appear in blood at increased levels in the presence of a neoplasm. Instead of detecting cancer, tumor cell markers are more useful in confirming diagnosis or monitoring therapy (Wilkinson, 2021). Tumor cell markers are ordered for patients with pancreatic cancer to help diagnose the origin of cancer if a patient presents with advanced widespread disease. It is also used to confirm pancreatic cancer diagnosis and monitor chemotherapy.

The tumor is in Stage III of the TNM system. Stage III is a locally advanced cancer in its late stages, with an invasion of the adjacent structures and involvement of several regional nodes (Wilkinson, 2021). However, there is no distant metastasis present. The pancreatic tumor has infiltrated the Wirsung duct and superior mesenteric vein. Ther4e is also the involvement of the perilesional node detected. The TNM classification helps assess a tumor’s size and determine if there is metastasis.

A malignant tumor is capable of metastasizing and causing death. The carcinogenesis phase occurs in four phases: tumor initiation, tumor promotion, malignant conversion, and tumor progression. Tumor promotion involves the selective clonal increase of initiated cells. Malignant conversion entails the preneoplastic cell transforming into one that expresses the malignant phenotype (Smith et al., 2020). Furthermore, tumor progression entails the expression of the malignant phenotype and the predisposition of malignant cells to become more aggressive over time. The tissue level affected by the patient is epithelial. This is because ductal cells of the pancreas form the epithelial lining of the branched tubes (Kane et al., 2020). The epithelial cells can invade through connective tissues and across the perineurium.

Past Medical History (PMH)

Patient is Diabetic, controlled with Metformin 500 mg by mouth twice a day, Lantus 15 units SC bedtime. Hypertensive, controlled with Olmesartan 20 mg by mouth once a day. Atrial Fibrillation, controlled with Rivaroxaban 15 mg by mouth once a day and bisoprolol 10 mg by mouth once a day.

Hb 12.7 g/dl; Hct 38.8% WBC 8.2; Glycemia 74mg/dl; Creatinine 0.8 mg/dl; BUN 9.8 mg/dl; AST 21 U/L ALT 17 U/L; Bil T 1.90 mg/dl; Ind 0.69 mg/dl; Dir 1.21 mg/dl.
Diagnostic test:

Endoscopic Ultrasound of the Pancreas. Solid mass in the head of pancreas 4 cms, infiltrating Wirsung duct. The solid mass impress to infiltrate the superior mesenteric vein. Perilesional node is detected, 1.5 cms, metastatic aspect. Fine needle aspiration (FNA) biopsy: Ductal adenocarcinoma.

You must proofread your paper. I discourage overutilization of direct quotes in DQs and assignments at the Masters’ level and deduct points accordingly.
As Masters’ level students, it is important that you be able to critically analyze and interpret information from journal articles and other resources. Simply restating someone else’s words does not demonstrate an understanding of the content or critical analysis of the content.
