Advanced registered nurses play a critical role in the current healthcare system in America. Advanced registered nurses enhance the access to care by addressing the shortage of physicians in the state. They also work with other healthcare providers and their patients to develop care interventions that utilize evidence-based data. In doing the above, advanced registered nurses practice in accordance with their scope of practice, standards, and philosophy. Therefore, this essay examines the scope of my future role as a nurse informaticist, professional organizations that will affect my scope, and evolving issues that are likely to affect my practice.

Scope of my Future Role as an Advanced Registered Nurse

My future role as an advanced registered nurse is nurse informaticist. Nurse informaticists are nurses who have advanced knowledge and skills in the use of informatics in healthcare. The scope of practice of nurse informaticists is broad. Accordingly, nurse informaticists integrate information management as well as analytical sciences into nursing science. The aim of integrating information from the above disciplines is to understand, define, and communicate the ways in which technology-related data can be used to inform nursing practice. The scope of nurse informatics also extends to the use of health technologies and their related data to make informed decisions in their organizations (Kleib & Nagle, 2018). Value-based healthcare is demonstrated by total joint replacement packages (Schardt et al., 2020). The orthopedic specialist follows surgical protocols when performing an elective joint replacement to achieve the best possible results. The advanced practice nurse can study the patient in an emergency room or office setting and offer early assistance for a quick recovery. Patient outcomes and payment outcomes improve, resulting in cost savings. The advanced registered nurse leader at the joint center collaborates with a group to develop data-driven protocols that produce positive results. The leader ensures that all staff members follow protocols in order to improve patient outcomes. The advanced practice nurse educator is in charge of teaching direct care nurses the clinical pathways that are necessary for the best results. My workplace has a Joint Center that operates in this manner to provide patient-centered, excellent, cost-effective, and efficient care. The goal is for nurse leaders to collaborate and think creatively in order to establish and sustain value-based care in their field.

A large number of patients will be discharged from hospitals over the next three to five years. Previously, many spinal cases would spend the night in the hospital. Patients who had post-operative cervical spinal fusions and discectomies and met the discharge criteria were discharged the same day, with nurse practitioner follow-up. Patients who meet all release criteria for total joints on the day of surgery are discharged home. In the healthcare system, home care is becoming more common, with advanced practice registered nurses providing ongoing office follow-up. Advanced practice registered nurses provide high-quality, low-cost care in a variety of settings, including hospitals and communities. Cost-cutting measures will be implemented in order to provide patients with dependable, safe care.

Advanced registered nursing graduates are entering the profession at dynamic time when roles and scope of practice are shifting based on developments in legislation and policy in response to the evolving needs of the health care system. Professional nursing organizations play an important role in making sure the perspectives of advanced registered nurses are heard, and in supporting nurse specialties in their efforts to expand their scope of practice and their full participation throughout the health care system.

As a prospective nurse leader, I would most likely join the American Organization for Nursing Leadership (AONL). The organization offers nurse leaders with the requisite tools to deliver competent and quality healthcare services. The goals and objectives of the organization entail ensuring that nurse leaders acquire innovative and expert nursing leadership aligns with my desire to provide quality healthcare services to patients. Moreover, the organization ensures that nurse leaders are ahead of emerging issues through their provision of competency-based education (Thomas et al., 2016). Further, the AONL ensures that healthcare transformation that will impact patient care occur through various undertakings. All the above lead to improved patient care, which is foundational to my personal philosophy. In my philosophy, I believe that patients should be given the utmost attention and everything that is done should lead to enhanced services regarding patient care.

Membership in the AONL will have remarkable influence on my leadership capabilities. As stated, the organization looks to impart nurse leaders with competency-based skills. Nurses continue to adopt greater responsibilities as leaders and managers in different healthcare settings due to current changes in legislations at state and federal levels. As the role of the nurse transforms from a physician’s assistant to an integral member of the life-saving team, the nurse leader role emerges. The scope of a nurse leader is wide but at the core of this responsibility is to guide nurses and ensure that they are complying to high standards of quality care and patient safety. In its report, the Institute of Medicine (IOM) noted the importance of nurses leading change and advancing health as essential ways of transforming the nursing profession and delivering quality and safe care to patients in diverse practice settings (Ducharme et al., 2017). Nurse leaders are responsible for ensuring safe delivery of care and are must be intimately familiar with the safety standards and protocols of the medical profession and their healthcare practice. Successful nurse leaders maintain and use evidence-based practices in their nursing team and settings. The implication is that as a nurse leader, one must be up to date and well conversant with different medical practices and protocols. Nurse leaders should be aware of different professional publications, be members of professional associations and use health notices to enhance care delivery (Ducharme et al., 2017). The care decisions and choices of nurse leaders should be informed by evidence-based research studies and public health concerns of the population and patients visiting their practice environment.  Nurse leaders are responsible for monitoring patient care to ascertain that members of their teams work toward optimum quality standards. Nurse leaders should consider patient concerns seriously and address them in the most effective way. As leaders, they should promote high quality care standards for each patient and their families. Successful nurse leaders focus on attainment of high quality experiences for both patients and nurses in their care setting.

Nurse leaders are members of professional organizations like the American Nurses Association and must adhere to state regulatory bodies’ requirements. For instance, the National Council of State Boards of Nursing (NCSBN) is a national entity which offer nurses with regulatory requirements at the federal level. The organization is responsible for development of state licensure examinations that adhere to current nursing practice. NCSBN provides state boards of nursing with current nursing practice programs to enhance collaborative approaches to different nursing issues and nurse leaders incorporate such new evidence into care practice among their nursing teams. The organization also provides boards with evidence-based practice content to guide nurse leaders in making decisions on regulatory issues that may impact their practice.

Nurse leaders should also understand the role of regulatory bodies like the Centers for Medicare and Medicaid Services (CMS), which is responsible for overseeing healthcare programs like the meaning application of electronic health records and value-based care and reimbursement protocols (CMS, n.d). The CMS has certain guidelines that nurse leaders should adhere to in order to get reimbursement. The society provides membership that includes seminars, webinars, reviews for certification, and a journal dedicated to perianesthesia care. New and current information is provided along with legislation that impacts the clinical areas of care. The organization encourages certification and offers reviews at various locations in the country (ASPAN, 2021). All three organizations have stated in some form that they are committed to nursing education and improving the care of each patient. This is my goal in furthering my education. I hope that my clinical knowledge will serve each patient well. I would also like to teach in order to produce nurses dedicated to professional improvement that serves our patients and communities.

The ANCC also offers many awards, scholarships and study aids to help promote education and the advancement of the nursing field.The American Nursing Informatics Association (ANIA) more specific to informatics. The ANIA seeks to “advance nursing informatics through education, research and practice in all roles and settings” (ANIA, 2021). Membership serves to be a good way to network, they offer continue education, give access to up to date research and the Journal of Informatics Nursing, have annual conferences, and have discussion forums for members. For membership requirements you just have to have a valid license and diploma in nursing, or being a current student of nursing, and you must pay the membership fee.The Society of Pediatric Cardiovascular Nurses (SPCN) is the smallest of the mentioned professional organizations but is most specifically related to my current role as a pediatric cardiovascular ICU nurse. The SPCN currently has about 400 members and serves as a great resource for those within the specialty to communicate with one another and share information and resources (SPNC, n.d.). The requirements are simply a membership fee. The SPCN serves a very small niche specialty, so it interests me to be able to connect with those within the same field.In my current work, care is so specific to this population, and the pediatric population as a whole is understudied. I feel the continued education and research related to nursing practice is of upmost importance. With advancements in technology and continued advancements in research and education I feel this will continue to serve and benefit the patient population. The three organizations of interest are the National Student Nurses’ Association (NSNA), the American Association of Colleges of Nursing (AACN), and the American Society of Perianesthesia Nurses (ASPAN). The NSNA dedicates the organization to the development of future nurses as they follow their educational path towards a career in nursing (National Student Nurses’ Association [NSNA], 2021). Additionally, the association provides scholarships to undergraduate students to complete their degrees. NSNA members can join their university and state nursing organizations and participate in leadership activities. The NSNA has biannual meetings that provide information relevant to current nursing, health care, and legislative issues (NSNA, 2021). The AACN represents nurses dedicated to academia (American Association of Colleges of Nursing [AACN], 2021). It establishes and implements educational standards for nursing students at any level of nursing education. The AACN guides nurses toward improving health care. Nurses are supported by the AACN in their education, research, and practice. The membership comprises nursing schools, faculty, and students (AACN, 2021). ASPAN is the organization that represents perianesthesia nursing. The group sets the standards of care and clinical guidelines for the care of patients in the preoperative and postoperative settings (American Society of PeriAnesthesia Nurses [ASPAN], 2021). The society provides membership that includes seminars, webinars, reviews for certification, and a journal dedicated to perianesthesia care. New and current information is provided along with legislation that impacts the clinical areas of care. The organization encourages certification and offers reviews at various locations in the country (ASPAN, 2021). All three organizations have stated in some form that they are committed to nursing education and improving the care of each patient. This is my goal in furthering my education. I hope that my clinical knowledge will serve each patient well. I would also like to teach in order to produce nurses dedicated to professional improvement that serves our patients and communities.

Camille, I concur with you that NSNA is an amazing organization since it is concerned with nursing career paths.  Therefore, the organization plays a crucial role in developing and mentoring future nurses.  To ensure that future nurses are competent to execute their obligations NSNA provides scholarships to undergraduate nursing students to complete their degrees (Brewington & Godfrey, 2020). The nursing agency ensures that future nurses are prepared to be successful.  Since its foundation, NSNA has facilitated most nursing students’ education in various universities.  The AACN is also an important nursing organization that is concerned with nurses’ education levels. Future Scope, Role, and Professional Obligations Paper

The nurse educator role is becoming more and more critical as healthcare in the U.S. evolves. There is a growing demand for skilled nursing professionals to meet the dynamic patient care needs (Booth et al., 2016). Besides, there is an increasing nursing shortage due to the high number of nurses retiring and the rising aging population with increased health needs. This essay seeks to discuss the scope of my future role as a nurse educator, professional organizations influential in advancing the scope of advanced nursing, and an evolving issue likely to affect my scope or role in the future.

Scope of My Future Role as an Advanced Registered Nurse

MSN-prepared nurse educators practice in various settings, such as nursing schools, community colleges, healthcare facilities, home care agencies, and long-term care facilities. As a nurse educator, I will be required to apply my clinical abilities and nursing experience to design a nursing curriculum, including content and courses, and evaluate nursing program outcomes (Booth et al., 2016). The nurse educator’s roles in the classroom setting include facilitating learning among nursing students and facilitating their development and socialization (Booth et al., 2016).  The scope of a nurse educator also entails engaging students in scholarship and functioning within the educational environment. The nurse educator instructs and trains new nurses on basic nursing knowledge and skills and experienced nurses on advanced techniques in the clinical setting. The nurse educator evaluates health care professionals for staff development activities (Booth et al., 2016). Besides, they collaborate with nurse supervisors and nurses to set improvement goals and meet educational objectives. The nurse educator designs, implements, and evaluates academic and continuing education programs for nurses at all levels.

Professional Nursing Organizations

American Nurses Association.

American Nurses Association is an organization that advocates for the interest of the 4 million registered nurses in the USA.  Its members come from all the fifty states and it began in 1896. The organization ensure that the nursing practice standards are enhanced, support a working environment that is safe and ethical, reinforces the nurses’ health and wellness and advocates on various healthcare issues (ANA, 2018). As a way of supporting registered nurses, they lobby Congress on issues that affect nurses. It is the mandate of the organization to expand the role of RN and APRN as well as source federal funding to support nurse’s training and education. Their duties also revolve around workplace violence, Medicare reform, and safe staffing. Members enjoy an online library that provides research and information, member-only webinars, top nursing journals and a platform to network with fellow nurses (ANA, 2018).

National League for Nursing

The National League Nursing is a premier organization that represent faculty and leaders in nursing education. NLN ensures that its members access network opportunities, nursing research grants, and professional development and public policy initiatives (NLN, 2019). The organization ensures that nurse educators’ accesses significant educational and leadership opportunities. As a member one receives premium discounts on NLN annual conference, leadership programming, scholarly writing retreat, NLN bookstore, faculty development webinars, and Certified Nurse Educator examination fees. Membership also allows one to enjoy complimentary subscriptions like the NLN peer-reviewed, research journal, NLN print newsletter, and data related to nursing schools, program enrollment, and graduation.  Lastly, one gets a chance to participate in NLN community in terms of networking with progressive leaders in nursing education, joining committee to enhance leadership skills, promoting nursing profession through the Government Affair Action Center, volunteering with a local NLN constituent league, applying for NLN grants and scholarships and exercising voting privilege (NLN, 2019).

Professional Nurse Educators Group

Professional Nurse Educators Group (PNEG) refers to a virtual network of educators whose goals is the lifelong learning of professional nurses. The group consists of faculty from nursing schools, staff development professionals, entrepreneurs and continuing nursing education professionals (Mead, 2019). The group aims at supporting and encouraging their members as a way of promoting best practices in the delivery of nursing care. The goal is achieved through the formation of a community among professional nurse educators, members’ collaboration, developing networking platforms, providing mentorship to new people in nurse education, holding annual conferences, honoring and recognizing members who have impacted the field of nursing professional development and bridging the gap between academic nursing education, continuing nursing education an staff development (Mead, 2019).

Future Paper on the Scope, Role, and Professional Obligation
Clinical nurse specialists are critical in influencing the modern world’s provision of high-quality healthcare. They bring unique expertise to clinical settings. Additionally, clinical nurse specialists apply their advanced knowledge and skills to aid in the achievement of high-quality outcomes for patients, their families, and communities at large (Hain & Fleck, 2014). Additionally, they serve as clinical mentors for other nurses and lead innovative change initiatives that improve healthcare systems’ ability to meet the diverse needs of the populations they serve. Clinical nurse specialists’ ability to facilitate these outcomes is attributed to their competencies. Consultation, collaboration, system leadership, research, direct care, and ethical decision-making are among the competencies (Foster & Flanders, 2014). As such, this research paper examines the scope of my future role as a clinical nurse specialist, the role of professional nursing organizations in advancing the scope, and controversial or evolving issues that are likely to affect the scope of my practice in the future.
The Purpose of My Future Career as a Clinical Nurse Specialist
Clinical nurse specialists practice within the confines of their state-mandated scope of practice. While some states allow clinical nurse specialists to practice independently of physicians, the majority require them to practice under the supervision of a physician. Clinical nurse specialists may practice independently in some states but must have a mutual or collaborative agreement with a doctor or physician in their area of specialization (DeNisco, 2019). Regardless of the state’s provisions, clinical nurse specialists have a broad scope of practice. It encompasses their role in diagnosing and treating their patients’ illnesses and diseases, prescribing pharmaceutical and non-pharmacological treatments, promoting wellness and recovery, and preventing disease and illness in the populations they serve.
Clinical nurse specialists conduct physical examinations of their patients in order to ascertain their health status. They obtain laboratory and diagnostic tests to confirm their patients’ diagnoses. Additionally, these nurses are expected to collaborate with other healthcare providers to assess, plan, implement, monitor, and evaluate care. Clinical nurse specialists’ presence on healthcare teams has been associated with improved outcomes, including shorter hospital stays, improved pain management, fewer complications, and a higher rate of patient satisfaction (American Nurses’ Association, 2016). As a result, the role of clinical nurse specialists is critical in enhancing healthcare quality metrics.
Clinical nurse specialists have a defined scope of practice. Licensing is one of them. Clinical nurse specialists should hold state-issued licenses to practice in their areas of specialization. Additionally, training in the area of specialization is required. Additionally, they must have received the required training to acquire the necessary knowledge and skills for providing specialized care. The respective nursing boards should accredit the institutions that provide training for them. Additionally, specialist nurses must possess a bachelor’s degree in nursing. A master’s degree in a specialty area is required, as are competencies in addressing patient needs and promoting the development of other nurses in an institution.

Introduction

In the United States, the position of advanced practice registered nurse (APRN) has steadily grown in importance. This is due to the significant role they play in bridging the gap between physicians and the population. It is a well-known fact that the number of practicing physicians is inversely proportional to the population they are intended to serve. To become an advanced practice nurse, one must complete a rigorous postgraduate program accredited in their specialty NUR 513 – Benchmark – Future Scope, Role, and Professional Obligations Paper. The current recommendation for this postgraduate degree is a PhD as the entry-level requirement for practice. The psychiatric-mental health nurse practitioner (PMHNP), for example, must then be certified by the American Nurses Credentialing Centre (ANCC) (ANCC, N.D.) and obtain a license to practice from the state in which they intend to work. Given that many socioeconomically disadvantaged communities cannot afford the expensive health services provided by the nation’s physicians, the role of advanced practice registered nurses (APRNs) has become quite important. These populations require affordable healthcare that is delivered to them in a convenient location. This is primary health care (PHC), and the frontline clinicians who will champion it in the future are advanced practice registered nurses (APRNs). This paper will examine the future roles and scope of the nurse practitioner (in this case a PMHNP), as well as the regulatory framework governing their practice, accreditation and credentialing, nursing professional organizations they can join, and a contentious issue pertaining to the practice of a PMHNP. Future Role Scope, Regulation, Accreditation, and Credentialing as a PMHNP

 

Currently, the role and scope of the nurse practitioner are heavily influenced by state legislation and regulations governing nurse practitioner practice. In states that guarantee full autonomy of practice (full practice authority or FPA), the PMHNP independently evaluates and diagnoses mentally ill patients, orders investigations, and interprets the results before prescribing appropriate treatment and arranging for follow-up (Maryville University, 2019). This will be the full scope of the PMHNP’s role in the future, as the movement toward universal full practice authority across states is irreversible. State legislation regulates the practice of PMHNPs and, by extension, all other APRNs. Different states have distinct APRN regulatory structures (AANP, 2018). In some of the fifty states, the regulatory structure is one of full practice authority. However, other states still have either a reduced practice or restricted practice regulatory structure (AANP, 2018). These last two regulatory structures are restrictive and prohibitive for nurse practitioners, preventing them from utilizing their knowledge and training to the fullest extent. As stated previously, the regulatory structure for full practice authority permits the nurse practitioner to practice without supervision or hindrance. Reduced practice, on the other hand, necessitates that the nurse practitioner enter into a collaborative agreement with a physician in order to practice effectively under them. In states with this type of APRN regulation, the nurse practitioner is restricted to performing a few tasks under the supervision of a physician (AANP, 2018). As of 2017, a total of 23 states have a regulatory structure for full practice authority. These include the states of Idaho, Iowa, Maryland, Hawaii, Washington, D.C., and New Mexico, among others. The remaining practices are either limited or restricted. States with restricted APRN practice include Ohio, Illinois, Wisconsin, Utah, Louisiana, etc. The worst states for APRN practice are listed last. These states have licensing laws that only permit restricted practice, giving APRNs very little room to grow. These states include, among others, Florida, Texas, Michigan, Georgia, and California (Maryville University, 2019; AANP, 2018).

 

However, the PMHNP and other APRNs have a promising future, and their traditional role, as outlined above, will expand. This is because physicians are leaving family medicine for superspecialties, leaving disciplines such as family medicine with severe manpower shortages. The cadre that will fill this vacancy is APRNs (Maryville University, 2019). After education and training, PMHNPs are expected to obtain certification before applying for a license to practice in the state in which they wish to work. In the case of the PMHNP, the American Nurses Credentialing Centre or ANCC is responsible for this certification (ANCC, n.d.). The only requirement for the graduate nurse practitioner is to sit for a clinical aptitude, knowledge, and skills examination. After passing the exam and receiving board certification (BC), the nurse practitioner’s official title changes to PMHNP-BC (ANCC, n.d.). Regarding accreditation, it is the specific course that the PMHNP takes at the university that must be accredited. Accreditation of programs and institutions occurs at both the regional and national levels (EDsmart, n.d.). Regional accreditation is preferred, however, because it facilitates the transfer of credits to another institution. The US Department of Education recognizes the Higher Learning Commission and the Northwest Commission on Colleges and Universities as regional accreditation agencies. In conclusion, the process begins with enrollment in an appropriately accredited postgraduate program. The nurse practitioner applies for board certification and takes the competency examination after completing the course. If successful, they apply for a practice license in accordance with the state’s licensing regulations. Depending on the state, they will then obtain a license for either full practice autonomy, a limited practice, or a restricted practice, as described previously.

 

Three Nursing Professional Organizations

 

Professional organizations are an essential element for the advanced practice nurse. This is especially true considering the difficulties presented by the diverse regulatory structures mentioned previously. Policies and their formulation are political processes by definition. If anything is to change, political processes require large numbers of people speaking with one voice. This is why PMHNPs must form professional organizations in order to advocate for their professional causes. Therefore, professional organizations provide a forum and platform for psychiatric-mental health nurses to voice their concerns (Catallo et al., 2014). These concerns include lobbying state legislators for policy changes leading to the granting of full practice authority in states where it is still lacking. Professional organizations also facilitate the exchange of professional ideas for the betterment of practice.

 

The American Psychiatric Nurses Association (APNA), the International Society of Psychiatric-Mental Health Nurses (ISPN), and the American Association of Nurse Practitioners are three of the most prominent professional organizations for the PMHNP (AANP). ISPN builds capacity through mentorship and idea exchange, while also facilitating the exchange of professional ideas. The APNA promotes the dissemination of knowledge and the professional development of its members (Adams &Black, 2016). In 2016, for instance, it had developed a brief online course to help its members close some identified knowledge gaps. American Psychiatric Nurses Association Transitions in Practice (ATP) was the name of the program (Adams & Black, 2016). The American Association of Nurse Practitioners (AANP) provides professional support and encouragement by granting Fellowship status to members who have contributed significantly to the nursing profession. Controversial Issue

 

Currently, the issue of evidence-based practice is the most contentious issue facing PMHNPs. In all of medicine, only psychiatry and mental health lag behind in the implementation of evidence-based practice. Research health care legislation that is being considered in your state. What impact might this legislation have on your future practice? What impact does it have on the collective practice of nurses in your state? How do you envision yourself potentially becoming involved in political advocacy for the profession in your future practice?

In California, Assembly Bill 890 (AB 890) Nurse Practitioner, scope of practice is one of the more recent bills passed. This was initially proposed during the pandemic when there was a shortage of physicians. With the passage of AB 890, Nurse Practitioners may be able to practice independently once certain provisions have been met. A nurse practitioner must first complete the transition to practice by practicing under one or more physicians for 3 full-time equivalent years of practice or 4600 hours. In order for nurse practitioners to expand their practice outside of those settings, the nurse practitioner must practice within the limits of their knowledge, experience, and certification in good standing for at least another 3 years after completion of the initial 3 years of transition to practice. The California Board of Nursing is expected to implement AB 890 on or before January 1, 2023 (California Board of Nursing, 2022).

Another related bill is SB 1375 which will increase access to affordable, quality abortion care and reproductive health services in California. It clarifies existing laws so that trained and experienced nurse practitioners can provide this critically needed care without physician supervision. With the shortage of primary care physicians in California, especially in the Latino, Black, and Native American communities, over 40% of the counties do not have clinics that provide abortion (California Association for Nurse Practitioners, 2022).

In my current role, I am actively involved in political advocacy that may affect our profession, our practice, and our patients. I have been through a number of propositions that may have adversely affected our dialysis population and I have been actively involved in advocating for or against these propositions. I see myself more actively involved in this in the future as well.
