Efficient medication management processes in psychiatric care and treatment are critical. Many patients are admitted to inpatient units for medication stabilization. Usually antipsychotic medications are being started for either the first time or a patient is being placed on a new medication and monitored for adverse or significant side effects.

Clozapine is used for patients who have not seen positive effects from first line antipsychotic or mood stabilizers. This drug requires ANC monitoring on a weekly basis as well as database registry in order to prescribe. This medication is highly controlled due to the life threatening side effects such as sever neutropenia (RxList, 2021).

 

A CDSS for clozapine monitoring is essential for psychiatric healthcare settings in order to protect patients from sever side effects. Required documentation must be entered on a weekly basis such as patient side effects tracking, ANC and WBC results when the provider is requesting to prescribe on a weekly basis. This information is then reviewed by the pharmacist with recommended dosage, once approved the order can then be filled.

A computer decision support system (CDSS) can be defined as an interactive software application that aids in the decision-making process by assisting clinicians, patients, or other decision-makers with information technology (Berrouiguet et al., 2017). When it comes to psychiatric drug prescribing, CDSSs are designed to help clinicians select the most efficacious and safe medication for their patients while considering factors such as patient preference, comorbidity, prior response to treatment, and cost. Clozapine is an atypical antipsychotic medication that has shown efficacy in treating refractory schizophrenia; however, it is associated with a number of serious side effects including agranulocytosis (Lau et al., 2021). CDSS for clozapine monitoring have been recently developed to help clinicians optimize clozapine treatment. One study showed that a CDSS for clozapine monitoring reduced the number of missed doses and improved patient adherence to clozapine treatment (Verma et al., 2021). The CDSS also helped identify patients who were at risk for developing side effects, allowing clinicians to take appropriate action. Thus, CDSS for clozapine monitoring can improve the safety and efficacy of this treatment.
